腹股沟小切口精索内静脉结扎+悬吊术的应用及临床意义

目的探讨在采用经腹股沟小切口精索内静脉结扎+悬吊术与应用腹腔镜治疗精索静脉曲张的手术方法中,腹腔镜是否较开放手术更具优越性。方法回顾性分析56例应用开放手术经腹股沟小切口精索内静脉结扎+悬吊术和50例应用腹腔镜精下索静脉高位结扎术的病例。结果两组病例在手术时间和术后住院时间上差异无统计学意义(P>0.05);住院费用差异有统计学意义;术后1年随访均无复发,两组间差异无统计学意义(P>0.05);精子质量均有明显改善,两组间差异无统计学意义(P<0.05);随访无睾丸萎缩。结论单侧精索静脉曲张,腹股沟小切口精索内静脉结扎+悬吊术仍值得首先推荐,针对双侧和复发的精索静脉曲张,则应优先考虑腹腔镜手术。

The Application and Clinical Significance of Internal Spermatic Vein Ligation Suspension Surgery with Small Groin Incision

Objective To investigate whether laparoscopic surgery has more advantages than internal spermatic vein ligation sus- pension surgery, an open surgery through small groin incision, in the treatment of varicocele. Methods 56 cases of patients with varicocele treated by internal spermatic vein ligation suspension surgery with small groin incision and other 50 cases of patients with varicocele treated by laparoscopic high ligation of spermatic vein were analyzed retrospectively. Results There were no statis- tical differences in operation time and postoperative hospitalization stay between the two groups（P〉0.05）. There was significant dif- ference in hospitalization costs between the groups. After 1-year follow-up postoperatively, no recurrence was found, the difference between the groups was not statistically significant（P〉0.05）; Sperm quality has improved obviously in both groups with no statisti- cally significant difference（P〈0.05）; And no testicular atrophy was found. Conclusion In the treatment of unilateral varicocele, in- ternal spermatic vein ligation suspension surgery with small groin incision is still worthy of first recommendation. For bilateral and recurrent varicocele, laparoscopic operation should be given the priority.
