关节镜下“8”字缝线与带线锚钉治疗胫骨髁间棘撕脱骨折的效果比较

目的比较关节镜下胫骨髁间棘撕脱骨折"8"字缝线与带线锚钉固定两种方法的疗效。方法笔者收集2007年1月~2013年9月收治的52例胫骨髁间棘骨折患者,均为Meyers-MckeeverⅡ型、Ⅲ型的患者,28例行"8"字缝线固定(A组),包括男性21例,女性7例;平均年龄24岁;24例行带线锚钉固定(B组),包括男性19例,女性5例;平均年龄22岁。比较两组伸膝功能、屈膝功能、关节稳定性及Lysholm关节功能评分。结果术后平均随访9.2个月。A组与B组伸膝功能有统计学差异(P=0.040.05),两组屈膝功能、关节稳定性及Lysholm关节功能评分无统计学差异。结论关节镜下"8"字缝线固定治疗胫骨髁间棘撕脱骨折的疗效优于带线锚钉固定。

Comparison of clinical effect of"8" shaped suture and anchor fixation under arthroscopy for avlusion fracture of the tibial intercondylar eminence

Objective To analyze the clinical efficacy of "8" shaped suture and anchor fixation for the treatment of avulsion fracture of tibial intercondylar eminence.Methods From Jan.2007 to Sept.2013,two differ-ent kinds of operational procedures were performed for 52 patients with avulsion fracture of tibial intercondylar emi-nence, who were classified into typeⅡ,typeⅢaccording to the Meyers-Mckeever.The 52 patients were divided in-to 2 groups:"8" shaped suture group(A group,n=28) including 21 males and 7 females with an average age of 24 years;anchor fixation group(group B,n=24) including 19 males and 5 females with an average age of 22 years. The extension function,flexion function,stability and Lysholm score of knee joints in the two groups were compared. Results All patients were followed up for 6 to 12 months ( average 9.2 months) .There were statistically signifi-cant differences between group A and group B for knee extension (P=0.04<0.05),while no statistically signifi-cant difference was found in the other functional aspects( P>0.05) .Conclusion "8" shaped suture is a prefera-ble surgical method for treating avulsion fracture of tibial intercondylar eminence compared to anchor fixation.
