肺癌候选抗原的血清反应分析及其联合检测的应用

摘    要:目的:通过重组cDNA表达文库血清学分析(SER-EX)技术筛选鉴定的5种肺癌候选抗原的血清学反应及其联合检测分析,对其能否作为肺癌早期诊断的血清学标志物进行评价,并提供一个肺癌早期诊断的新方向.方法:采用重组克隆抗原表达技术扩增5种肺癌候选抗原分别是X5:二羟基苯甲酸内脂结合蛋白1配偶体(POB1);X18:DNA拓扑异构酶Ⅱα(Top2A);X44:中枢神经系肽酶Ⅰ(TPP1);X48:热休克蛋白90а(HSP90а);X52:真核翻译延长因子1γ(eukaryotic translation elongation factor 1 gamma)],免疫印迹法检测肺癌患者血清、正常人血清中自身抗体的反应,流行病学方法对其检测结果进行评价.结果:POB1,Top2A,HSP90а3个肺癌候选抗原克隆在肺癌患者血清和正常人血清中的相应IgG抗体阳性率差别有统计学意义(P〈0.05);3个阳性反应抗原抗体反应联合分析时,能够在保持特异度等基本不变的前提下,明显提高诊断肺癌的灵敏度,具有较大的临床诊断价值.结论:3个抗原可作为肺癌的早期诊断的候选标志物,为肺癌的早期诊断提供了一个新的方向.

Abstract:AIM:To investigate sero-reaction of 5 lung cancer candidate antigens which were identified by using SEREX(serological analysis of recombination cDNA expression libraries) and that of further combinational detection,to evaluate whether or not these antigens could be used as markers in early diagnosis of lung cancer and to provide a new direction of early diagnosis of lung cancer. METHODS:Five lung cancer candidate antigens (X5:partner of Ral-binding protein 1,POB1; X18:topisomeraseDNA]Ⅱalpha 170 kDa,Top2A; X44: tripeptidyl peptidase I ,TPP1; X48: heat shock protein 90 kDa alpha, HSP90alpha; X52: eukaryotic translation elongation factor 1 gamma ) were expressed from each corresponding cDNA clones and were subse- quently used in immunoblotting analysis for detecting antibody responses with sera of lung cancer patients and normal human. The testing results were evaluated by epidemiological method. RESULTS: The positive rate of IgG antibodies of 3 antigens (POB1, Top2A and HSP90alpha) in lung cancer patients was obviously higher than that in normal controls ; When the combinational analysis of these 3 antigen-antibody systems was performed, the sensitivity for detection of lung cancer was significantly increased with invariable specificity on the whole, suggesting that these 3 antigens may have a higher value to diagnose lung cancer early. CONCLUSION: This study can provide candidate markers in early diagnosis and combinational detection of lung cancer, and also highlight a new direction of early diagnosis of lung cancer.
