内括约肌减张缝合修补术在肛瘘手术中的应用

目的寻求肛瘘手术中更好的保护肛门功能的方法,减少术后漏气漏液发生率,设计了内括约肌减张缝合修补用于低位肛瘘的治疗。方法选择60例低位肛瘘患者,随机分为观察组30例,采用内括约肌减张缝合修补术,对照组30例,采用常规瘘管切开术。结果观察组患者创口愈合时间缩短,漏气漏液减少,术后随访无应急性排便失禁。结论该手术方法具有创伤小,疗程短,远期效果好,并发症少等优点,值得临床推广应用。

Relief Incision and Anastomosis of Internal Sphincter in Treatment of Anal Fistula Diseases

Objective To obtain a proper procedure of protecting internal sphincter in the operation for anal fistula diseases,and to raise the curative effects,the relief incision and anastomosis of internal anal sphincter was designed for application in treatment of corresponding low anal fistula.Methods Sixty patients with low anal fistula were included in the study,of which 30 cases underwent the relief incision and stitch of internal anal sphincter(the study group),while 30 cases underwent fistula incision(controlled group).The operation was that,we dissociated partial internal anal sphincter,divided it into two sections,cut them as "Z",anastomosed the incision,then the incised wound was opened for drainage.Results The incised wound healed fast,and less urine was retained.There was no postoperative contingent incontinence of the rectal.Conclusions This operation has such advantages as small incised wound,short treatment course,good long-term curative effect,less complications,and is worth spreading and application.
