雌激素受体亚型在子宫内膜异位症在位内膜及小鼠模型中的表达及意义

摘    要: 目的探讨雌激素受体不同亚型(ERа和ERβ)在子宫内膜异位症(EM)的在位内膜及小鼠模型的异位病灶中的表达及意义。方法收集子宫内膜异位症患者20 例(EM 组)和非子宫内膜异位症患者20 例为对照组(NEM 组),分别利用0.2,0.4,0.6 g 梯度量化子宫内膜建立小鼠模型,5 d后获取异位病灶组织,应用免疫组化方法检测ERа和ERβ蛋白的表达。结果EM 组及NEM组种植率比较:0.2 g 的种植率分别为60%和15%,差异有统计学意义( P< 0.05);0.4 g 种植率分别为100%和85%,0.6 g种植率分别为100%和100%,差异无统计学意义( P> 0.05)。异位病灶体积比较,EM组各组病灶均大于NEM组( P< 0.05)。ERа蛋白表达上,在位内膜中EM组比NEM组阳性率高,在EM组中在位内膜阳性率均比同组各异位病灶高,差异有统计学意义( P< 0.05)。NEM 组在位内膜与异位病灶ERа蛋白表达比较,差异无统计学意义( P> 0.05)。ERβ蛋白表达上,在位内膜及异位病灶中EM组比NEM 组阳性率高,差异均有统计学意义( P< 0.05),而两组中在位内膜阳性率均比同组各异位病灶低,差异有统计学意义( P< 0.05)。两组在位内膜ERβ蛋白表达均比同组相应各组异位病灶少,差异有统计学意义( P<0.05)。结论子宫内膜异位症患者在位内膜具有更强的异位种植能力,在位内膜中ERа和ERβ均高表达,雌激素受体高表达在子宫内膜异位症发生发展中的作用可能以β亚型的作用为主。

Abstract:Objective To approach the expression and significance of estrogen receptor subtypes (ERаand ERβ)in the eutopic en-dometrium of patients with endometriosis (EM)and the ectopic focus of on the mice model. Methods 20 cases of EM patients was col-lected as the EM group and 20 cases of NEM patients (CINⅢ)was collected as control (NEM group). The gradient quantifiable mice model was eatabilished estabilished by implanting the endometrium of 0.2 g,0.4 g and 0.6 g,respectively,and the ectopic focus were ob-tained 5 days later. The protein expression of ERаand ERβwas detected by immunohistochemistry. Results The implantation rate of the EM group compare to the NEM group60% and 15% in the 0.2 g group,respectively,there was statistical significance diference between the two groups(P < 0.05);100% and 85% in the 0.4 g group and both 100% in the 0.6 g group,there was no statistical significance diference between the two groups( P>0.05). The volume of the ectopic focus in the EM group was larger than that in the NEM group (P < 0.05). The expression of ERаin the eutopic endometrium of the EM group was higher than that of the NEM group(P < 0.05);In the EM group,the expressions of ERаin the eutopic endometrium was higher than that in the ectopic focus(P < 0.05),there was statistical significance
diference between the two groups(P < 0.05);while in the NEM group,there was no significant difference of the ERаprotein expressions be-tween the eutopic endometrium and the ectopic focus. there was no statistical significance diference between the two groups( P>0.05). The protein expression of the ERβin the eutopic endometrium and the ectopic focus of EM group was higher than them that of the NEM group(P < 0.05),respectively,there was statistical significance diference between the two groups(P < 0.05),and in both the EM and NEM group,the pro-tein expression of ERβin the eutopic endometrium was lower than that in the ectopic focus in both the EM and NEM group,there was statistical significance diference between the two groups(P < 0.05). Conclusion The eutopic endometrium of EM group has had a much greater implan-tation capability,the expression of ERаand ERβare were both higher in eutopic endometrium of the EM group,and the role of the estrogen re-ceptor in the pathogenesis and progresion of the EMwas probably determined by the ERβsubtype.
