妇科千金片治疗慢性盆腔炎有效性及安全性的系统评价

目的]系统评价妇科千金片治疗慢性盆腔炎的有效性和安全性。方法]检索中文全文期刊数据库(CNKI)、万方数据库(WF)、维普数据库(VIP)、中国生物医学文献数据库(Sino Med)及Pubmed、EMbase、The Cochrane Library数据库,搜集关于妇科千金片联合西药治疗慢性盆腔炎的随机对照试验(RCT),检索时限均以建库至2017年2月。由两名评价者独立选择试验、提取资料、交叉核对和进行方法学质量评估。统计分析采用Rev Man 5.3版软件进行。结果]最终纳入12个RCT,共1 291例慢性盆腔炎患者。Meta分析结果显示:1)妇科千金片可以明显提高治疗慢性盆腔炎的总有效率(RR=1.21,95%CI1.15,1.27])。2)妇科千金片对慢性盆腔炎子宫抬举痛有明显的改善,MD=-4.33,95%CI(-4.91,-3.75),P0.000 01]。3)妇科千金片对慢性盆腔炎腹痛有明显的改善MD=-4.02,95%CI(-4.35,-3.69),P0.000 01]。4)妇科千金片对于白带异常有明显改善MD=-3.40,95%CI(-5.07,-1.74),P0.000 01]。5)不良反应:主要为皮疹、口苦涩感、恶心呕吐,无统计学意义。结论]现有研究显示,妇科千金片联合西药治疗慢性盆腔炎的总有效率较单纯使用西药明显提高,对慢性盆腔炎子宫抬举痛、腹痛、白带异常有明显的改善,并且无明显不良反应。由于本系统评价纳入RCT多为较低质量的小样本研究,需要更多高质量的研究进一步验证其疗效。

A systematic review of the efficacy and safety of Gynecologic Qianjin tablets in the treatment of chronic pelvic inflammatory disease

Objective] To systematically evaluate the effectiveness and safety of Gynecologic Qianjin tablets in the treatment of chronic pelvic inflammatory disease, seven databases were searched for studies on the safety and effectiveness of Gynecologic Qianjin tablets.Methods] Search database of CNKI, WF, VIP, SinoMed, Pubmed, EMbase, The Cochrane Library, collect about Gynecologic Qianjin tablets randomized controlled trials for the treatment of chronic pelvic inflammatory disease. Retrieval time limit are built from the library until February 2017. Two evaluators independently selected trials, extracted data, cross-checked and methodological quality assessments.Statistical analysis was performed using RevMan version 5.3 software.Results] Eventually included 12 RCTs, a total of 1 291 patients with chronic pelvic inflammatory disease. Meta analysis showed that:1) Gynecologic Qianjin tablets can significantly improve the total effective rate of chronic pelvic inflammatory disease,(RR=1.21, 95% CI1.15, 1.27]),2) Gynecologic Qianjin tablets can significantly relieve chronic pelvic inflammatory uterine lift pain,MD=-4.33,95% CI(-4.91,-3.75),P<0.000 01],3) Gynecologic Qianjin tablets significantly improved abdominal pain chronic pelvic inflammatory disease,MD=-4.09,95% CI(-4.35,-3.69),P<0.000 01],4) Gynecologic Qianjin tablets significant improvement for abnormal vaginal discharge,MD=-3.40,95% CI(-5.07,-1.74),P<0.000 01],5)Adverse reactions:Mainly including rash, mouth bitter astringent, nausea and vomiting. There is no statistically significant.Conclusion] The existing study shows that the total effective rate of chronic pelvic inflammatory disease of Gynecologic Qianjin tablets compared with the control group was significantly improved, the pain of chronic pelvic inflammatory uterine lift significantly improved. And no obvious adverse reactions. Because this system evaluates the inclusion of RCTs for low-quality, small sample studies, more high-quality studies are needed to further validate their efficacy.
