肺癌患者CD、P53、P16、nm23、EGFR和CEA表达及其与预后的关系

摘    要:目的 探讨组织蛋白酶D(CD)、表皮生长因子受体 (EGFR)、转移抑制基因nm2 3、抑癌基因P1 6、P5 3、癌胚抗原 (CEA)在非小细胞肺癌 (NSCLC)的表达及临床意义。方法 采用链霉抗生物素蛋白 过氧化物酶 (S P)快速免疫组化法检测了 6 4例肺癌标本六种蛋白的表达。结果 肺癌组织P5 3、CD、EGFR、nm2 3、CEA和P1 6表达的阳性率分别为 76 6 %、6 5 6 %、6 7 2 %、6 0 9%、76 6 %和5 1 6 % ,P1 6蛋白阴性表达率达 48 4% ,CD、CEA和EGFR表达与肺癌患者临床分期 (P <0 0 5、P <0 0 5、P <0 0 1 )、淋巴结转移 (P <0 0 1、P <0 0 5、P <0 0 5 )密切相关 ,后者亦与细胞分化程度有关 (P <0 0 1 )。COX比例风险模型分析和Kaplan Meier存活曲线分析显示CD和EGFR是影响肺癌病人预后的重要因素 (P <0 0 5 )。结论 在NSCLC中nm2 3、P1 6、CD、P5 3、CEA和EGFR异常改变的频率很高 ,它们在NSCLC的发生、发展中可能起促进作用。CD和EGFR可作为评价NSCLC患者的预后指标之一

Abstract:Objective To study clinical significance of the expressions of cathepsin D(CD), epidermal growth factor receptor(EGFR), nm23, P16, P53 and carcinoembrynic antigen(CEA) in patients with non small cell lung cancer.(NSCLC). Methods A rapid immunohistochemical method (streptavidin peroxidase conjugataed method SP) was used to detect the expressions of the six proteins in the lung cancer samples from 64 cases of NSCLS. Results The positive expression rates of P53, CD, EGFR, nm23, CEA and P16 were 76 6%, 65 6%, 67.2%, 60 9%, 76 6% and 51 6% respectively in the lung cancer tissues. The negative expression rate of P16 protein was 48 4%. The expressions of CD, CEA and EGFR were significantly correlated to the stages of the disease ( P <0 05, P <0 05, P <0 01) and lymph node metastasis ( P <0 01, P <0 05, P <0 05) in the lung cancer patients, and that of EGFR also had a marked correlation to the cell differentiation ( P <0 01). The multivariate analysis with COX hazard model and Kaplan Meier method showed that the expressions of CD and EGFR were the most important indicators in prognosis of the disease. Conclusion The abnormal expressions of all the six factors are very high in the NSCLC patients, and they may play promoting roles in the incidence and development of NSCLC. The expressions of CD and EGFR are the prognostic indicator of NSCLC.
