120例颅底肿瘤临床分析

目的探讨颅底头颈肿瘤手术的适应证、手术入路、多学科合作、修复及并发症等相关问题。方法回顾分析1995~2004年中国医学科学院中国协和医科大学肿瘤医院外科处理侵及颅底肿瘤120例的经验。结果良性肿瘤25例手术后效果良好,无手术死亡;各种类型恶性肿瘤95例,随诊3年以上者生存率64.6%,无瘤生存46.2%。恶性肿瘤术后住院期间死亡2例,手术死亡率2.1%(2/95)。手术并发症发生率21.6%(26/120)。结论术前对颅底病变和手术适应证充分评估,制订周密的手术方案,采取恰当的手术入路,同时相关学科间密切配合,可望进一步提高颅底肿瘤的治疗效果。

Analysis of 120 cases of skull-base tumors

Objective To analyze and evaluate the operative indications and approaches,the multidisciplinary collaborations,repairing,and complications for the treatment of skull-base tumors.Methods The clinic data of 120 cases of skull-base tumors,including 25 cases of benign tumors and 95 cases of malignant tumors,treated surgically at The Cancer Hospital of Chinese Academy of Medical Sciences from January 1995 to December 2004 were retrospectively reviewed and analyzed.Results All patients with benign tumors had good prognosis after surgery.(2.1%) cases with malignant tumors died during operation,and(21.6%) had surgical complications.65 cases with malignant tumors had been followed up after treatment,and 42((64.6%)) of them survived more than three years,including 30((46.2%)) tumor-free survivals.Conclusion Carefully evaluating and analyzing skull-base lesions and surgical indications before operation,adopting suitable surgery approaches,and having close multidisciplinary collaborations are very important factors to improve the therapeutic efficacy of the disease.
