同种异体骨髓间充质干细胞辅助自体骨软骨镶嵌成形术修复骨软骨缺损的研究

摘    要:目的探讨基于同种异体骨髓间充质干细胞(BMSCs)组织工程技术辅助自体骨软骨镶嵌成形术(Mosaicplasty)修复骨软骨缺损及促进"死区"的整合效果。方法首先从一只新西兰大白兔的骨髓内提取骨髓间充质干细胞,体外培养到第三代,用Pluronic F-127复合骨髓间充质干细胞,然后制作骨软骨缺损模型,先用自体骨软骨柱填充缺损,再用复合骨髓间充质干细胞的Pluronic F-127填充间隙作为实验组,单纯Pluronic F-127填充间隙的作为对照组,在不同时段运用组织学及免疫组织化学进行检测,同时进行统计学分析。结果两组相比各时间段的自体骨软骨柱均无松动、脱出及凹陷。实验组"死区"的修复组织逐渐演化为透明软骨,软骨细胞排列逐渐变得规则。4、8、12、16周O’Driscoll、Keeley and Salter组织形态学评分实验组分别为(14.00±1.00)、(16.75±1.71)、(18.00±0.82)和(20.50±1.29)分,对照组分别为(7.67±0.58)、(8.00±0.82)、(8.50±0.58)和(9.00±0.82)分。对结果进行析因试验资料方差分析,实验组的评分均高于对照组,不同处理间差异有统计学意义(F=419.302,P=0.000),左右侧之间差异无统计学意义(F=0.020,P=0.890),处理后不同时间段之间差异有统计学意义(F=13.184,P=0.000),不同处理间和不同时间段间的交互效应差异有统计学意义(F=4.982,P=0.015),其他组合的交互效应差异无统计学意义。结论同种异体骨髓间充质干细胞辅助自体骨软骨镶嵌成形术修复骨软骨缺损,可促进自体骨软骨镶嵌成形术中存在的"死区"的整合,改善修复效果,有望成为修复骨软骨缺损的有效方法。
