瑞芬太尼与芬太尼在小儿先天性心脏病手术麻醉中的应用比较

目的观察瑞芬太尼和芬太尼用于小儿先天性心脏病术中麻醉的有效性、安全性、术后不良反应及恢复情况。方法回顾性分析60例先天性心脏病矫正手术患儿,其中瑞芬太尼组30例,芬太尼组30例。观察两组患儿的麻醉拔管时间、麻醉苏醒及不良反应等情况。结果瑞芬太尼组患儿用药量多于芬太尼组用量,患儿拔管和苏醒时间前组短于后组,差异有统计学意义(P<0.05);在不良反应方面,两组比较差异无统计学意义(P>0.05)。结论瑞芬太尼用于小儿先天性心脏病手术中安全有效,术后拔管时间短、苏醒快,其麻醉和镇痛效果优于芬太尼,是目前临床上一种较理想的麻醉镇痛药。

Comparison of Application of Remifentanil and Fentanyl in Pediatric Congenital Heart Disease Surgery Anesthesia

Objective To investigate and compare the efficacy,safety,postoperative untoward reaction and recovery between remifentanil and fentanyl in pediatric congenital heart disease surgery anethesia.Methods Retrospective analysis was performed on 60 cases of pediatric heart corrective surgery,including 30 cases in remifentanil group and 30 cases in fentanyl group.Anesthesia extubation time,anesthesia awakening time and postoperative untoward reaction were observed.Results There was statistically significant(P<0.05)in dosage of medicine,the time of waking and postoperative extubation between the two groups.Dosage of medicine applied in remifentanil group was more than fentanyl group and the time of waking and postoperative extubation in former group was shorter than the latter.There was no significant difference of postoperative untoward reaction between two groups(P>0.05).Conclusion Remifentanil for pediatric congenital heart disease operation is safe and effective,postoperative extubation time short,quick recovery,with their anesthetic and analgesic effect better than that of fentanyl,therefore remifentanil is an ideal clinical narcotic analgesics at present.
