甲型H1N1流感确诊病例的临床分析

目的分析探讨甲型H1N1流感确诊病例临床特点及肺部影像学特点,以提高对甲型H1N1肺炎的认识与诊断水平。方法对河北省480例甲型H1N1流感并发重症肺炎患者的一般临床资料、症状、体征、辅助检查、诊断及结果进行分析。结果 480例患者中455例患者以发热为首发症状,61例患者出现咯血;312例患者肺部可闻及啰音。肺CT主要表现为双肺多发的斑片影和磨玻璃影,影像学表现变化以进展-逐渐吸收模式多见。结论甲型H1N1流感肺部病变多出现于双侧下肺,可表现为单样性或多样性病灶,病变的发展变化过程较为迅速,影像学检查在病变的诊断和治疗及疗效评价中具有重要价值。

The Clinical Features of Confirmed Cases of Influenza A H1N1

Objective To analyze the clinical features and chest-image characteristics of confirmed cases of influenza A H1N1,and to improve the level of awareness and diagnosis of influenza A H1N1 pneumonia.Methods The clinical data,symptoms,signs,laboratory examinations,diagnosis,and results of 480 patients with influenza A H1N1 complicated by severe pneumonia in Hebei province were analyzed.Results Among the 480 patients,455 cases of patients had fever as the first symptom,61 patients had hemoptysis;312 cases of patients had pulmonary rales.The lung CT image mainly showed multiple lung patchy foci and ground-glass opacification.The main imaging changes were progress-gradual absorption mode.Conclusion The lung disease of influenza A H1N1 is often seen in the bilateral lower lung,which can present as a single or diversified lesions;lesions development and change is relatively rapid,and imaging tests have great value in the diagnosis and treatment and clinical evaluation.
