窄带成像技术与碘染色对早期食管癌及癌前病变的诊断价值和对比分析

目的研究窄带成像技术(NBI)对早期食管癌及癌前病变的诊断价值。方法 78例常规内镜下食管黏膜病变患者分别用NBI、碘染色法观察病变形态、范围并活检组织作病理学分析。结果 NBI和碘染色内镜显示食管病变程度及范围明显优于常规内镜,NBI和碘染色对病灶检出率均较高(88%和90.2%),高级别上皮内瘤变主要表现为NBIⅠ级(82.4%)和碘染色Ⅰ级(89.1%),低级别上皮内瘤变主要表现为NBIⅡ/Ⅲ级(63.2%)和碘染色Ⅱ/Ⅲ级(73.4%)。结论 NBI和碘染色对早期食管癌及癌前病变均具有较高检出率,NBI可以替代碘染色内镜。

A Comparative Study of Narrow Band Imaging and Iodine Staining in the Diagnosis of Early Esophageal Cancer and Precancerous Lesions

Objective To explore the value of narrow band imaging(NBI)in early diagnosis of esophageal cancer and precancerous lesions.Methods The morphology change was observed with NBI and iodine staining in 78 patients with esophageal mucosa,and pathological biopsy were performed.Results NBI and iodine staining endoscope were superior to conventional endoscope on the degree and extent of esophageal lesions.With NBI and iodine chromoscopy,the incidence rate of lesions was 88% and 90.2%,respectively.For iodine staining,89.1% Grade Ⅰ was high grade intraepithelial neoplasia(HGN),and 73.4% Grade Ⅱ/Ⅲ was low grade intraepithelial neoplasia(LGN).For NBI,82.4% Grade Ⅰ was HGN,and 63.2% Grade Ⅱ/Ⅲ was LGN.Conclusion There is a high detection rate in early diagnosis of esophageal cancer and precancerous lesions by NBI endoscopy and iodine staining endoscopy.NBI endoscopy could replace iodine staining endoscopy.
