不典型性肺结核的影像学诊断价值

目的探讨不典型性肺结核的影像学特点,比较X线诊断与CT诊断对该病病变的诊断价值。方法回顾性分析2010年1月～2011年6月本防治中心收治的不典型性肺结核患者35例的X线及CT检查资料,总结其X线、CT诊断表现,并将检查结果与最终临床治疗确诊结果进行对比。结果经抗结核临床治疗结果与症状对比分析,35例不典型性肺结核患者X线正确诊断27例,不能确诊或漏诊8例,正确诊断率为77.14%;CT正确诊断33例,不能确诊或漏诊2例,正确诊断率为94.29%。X线、CT检查结果均与临床确诊结果呈正相关（0〈r〈1）,且CT检查正确诊断率明显高于X线诊断,差异有统计学意义（χ2=1.769,P〈0.05）。结论 CT对不典型性肺结核的正确诊断率明显高于X线诊断,详细了解并掌握不典型性肺结核患者的X线及CT多样性表现特征,并结合患者的临床表现进行诊断,可显著提高正确诊断率,降低误诊、漏诊的发生

Imaging diagnosis value of atypism tuberculosis

Objective To study the image characteristic of atypism tuberculosis,and to compare the diagnostic value of X-ray and CT.Methods 35 cases X-ray and CT examination material from January 2010 to June 2011 were retrospectively analyzed,the X-ray,CT performance were sum up,the results and the eventually diagnosed clinical treatment results were compared.Results All of the 35 patients,27 cases were correctly diagnosed by X-ray,8 cases were not diagnosed,the correct diagnosis rate was 77.14%;33 cases were correctly diagnosed by CT,2 cases were not diagnosed,the correct diagnosis rate was 94.29%.X-ray,CT examination results were both positively correlated with clinical diagnosis results(0 < r < 1),and CT correct diagnosis rate was higher than that of X-ray,the difference was statistically significant(χ2=1.769,P < 0.05).Conclusion CT diagnosing on atypism tuberculosis is better than X-ray diagnosis,detailed understanding and grasping of the multiformity characteristics of X-ray and CT,and combining with the clinical manifestations of patients,can significantly improve the correct diagnosis rate,reduce the occurrence of missed diagnosis.
