米非司酮配伍米索前列醇终止10～15周妊娠的临床分析

目的探讨米非司酮配伍米索前列醇的不同给药方法对主观有意终止妊娠妇女的临床效果。方法将300例患者随机分为研究组和对照组,各150例,两组患者均应用米非司酮联合米索前列醇终止妊娠,但用药方法有所区别。对照组患者于每日早晚7∶00均口服米非司酮,连续服用3 d,第4天早上7∶00,给予米索前列醇口服。研究组患者于第1天晚上7∶00一次性服用米非司酮,后于第2天早上7∶00服用米索前列醇,并使用氯化钠注射液将米索前列醇湿化后,放入患者阴道后窟窿处。用药后24 h持续观察和随访患者,观察孕囊排出时间、阴道流血情况和副作用,并进行疗效评价。结果研究组患者完全流产率明显高于对照组,不完全流产率和流产失败率明显低于对照组（P〈0.05）;研究组患者孕囊排出平均时间为（10.32±3.61）d,对照组为（15.81±5.63）d（P〈0.05）;研究组阴道出血量近似于经量的患者比例明显高于对照组（P〈0.05）;研究组患者发热、四肢麻木、腹泻、恶心、呕吐的发生率明显低于对照组（P〈0.05）。结论采用低频次、稍大剂量联合阴道给药的方法联合应用米非司酮及米索前列醇终止10～15周妊娠具有更优的临床有效性和安全性,适合于临床推广应用

Clinical analysis of 10-15 weeks of pregnancy termination with Mifepristone and Misoprostol

Objective To discuss the clinical effects of different medication methods with Mifepristone and Misoprostol in women of pregnancy termination for 10-15 weeks.Methods 300 patients were distributed into study group with 150 cases and control group with 150 cases.Two groups were both treated by Mifepristone and Misoprostol,but with different medication methods.Control group took Mifepristone at 7∶00 daily morning and evening with 3 days,then took Misoprostol at 7∶00 in the 4th morning.Study group took Mifepristone at 7∶00 in the 1st evening and took Misoprostol at 7∶00 in the 2nd morning,and was put Misoprostol into patients’ vagina.The gestational sac time,vaginal bleeding and side effects were observed.Results The complete abortion rate of study group were significantly higher than control group,incomplete abortion rate and abortion failed rate of study group were significantly lower than control group(P < 0.05).The average discharge time of gestational sac of study group was(10.32±3.61) d and control group was(15.81±5.63) d,the two groups had significant difference(P < 0.05).The rate of vaginal bleeding approximate menstrual flow in study group was significantly higher than control group(P < 0.05).The incidence rate of fever,limb numbness,diarrhea,nausea,vomiting in study group were significantly lower than control group(P < 0.05).Conclusion The method of low times,larger doses combined with vaginal delivery of Mifepristone and Misoprostol is more effective and safe in 10-15 weeks’ pregnancy termination,and suitable for clinical application.
