子宫动脉阻断术在腹腔镜下子宫肌瘤切除术中的临床效果评价

目的评估子宫动脉阻断术在腹腔镜下子宫肌瘤切除术中的临床效果。方法将42例腹腔镜下子宫肌瘤切患者随机分为观察组(22例)和对照组(20例),观察组行腹腔镜下子宫动脉阻断术后子宫肌瘤切除术,对照组在瘤体周围注入催产素后行腹腔镜下子宫肌瘤切除术。比较两组患者的手术时间、术中出血量、术后恢复情况,门诊随访观察子宫肌瘤复发、月经改善及生育情况。结果两组患者手术时间比较差异无统计学意义(P>0.05),术中出血量比较差异有统计学意义(P<0.05),所有患者症状均改善,无一例复发和术后并发症。平均随访18个月均未见肌瘤复发,3例未生育者正常妊娠至足月。结论腹腔镜下子宫动脉阻断术后子宫肌瘤切除术是有效减少术中出血,保留子宫,保留生育功能的新方法。

The Clinical Evaluation of Uterine Artery Blocking in Laparoscopic Hysteromyoma Resection

Objective To evaluate clinical effects of uterine artery blocking method in laparoscopic hysteromyoma resection.Methods 42 patients of laparoscopic hysteromyoma resection were randomly divided into trial group(22 cases)and control group(20 cases).Uterine artery blocking method were used in laparoscopic hysteromyoma resection in the trial group,laparoscopic hysteromyoma resection after injection of oxytocin around the tumor were done in the control group.Two groups were compared by operation time,peri-operative bleeding and postoperative recovery.Outpatient service follow-up of uterine fibroids recurrence,menstrual improvement and fertility.Results The operation time of two groups was not significantly different(P>0.05),the peri-operative bleeding was significantly different(P<0.05).The symptoms of all the patients were improved,no recurrence or complication after surgery occurred.Fibroids relapse was not seen in the 18 months follow-up,3 cases who did not bear before had normal pregnancy to term.Conclusion Uterine artery blocking method in laparoscopic hysteromyoma resection is a new method which can effectively reduce peri-operative bleeding,keep the uterus and keep reproductive function.
