肉瘤样肾细胞癌5例临床病理学观察

目的：观察肉瘤样肾细胞癌(SRCC)的临床病理特征。方法：对5例SRCC的临床病理特征、免疫表型及影响预后的因素进行评价。结果：组织学检查分为双相性SRCC4例，由癌和肉瘤样成分组成；单相性1例，由差分化的肉瘤样梭形细胞、单核细胞与破骨细胞样多核巨细胞组成。5例肉瘤样成分角蛋白阳性，4例波形蛋白阳性。结论：SRCC为高度恶性的上皮性肿瘤，形态特征为癌细胞具有双重分化，临床分期是影响患者预后的重要因素。

Sarcomatoid renal cell carcinoma:A clinicopathologic study of 5 cases

Objective:To study the clinicopathologic features of sarcomatoid renal cell carcinoma(SRCC).Methods:The clinical data,immunophenotypes and the prognosis related factors were analysed in 5 cases of SRCC.Results:The tumor was divided into two histologic subtypes:biphasic type(4 cases) and monophasic type(1 case).Biphasic RSCs were mixed with carcinoma and sarcomatoid elements.Monophasic RSC was composed of poorly differentiated sarcoma like spindle cells,mononucleate cells and osteoclast like multinuclear giant cells.Five cases of sarcomatoid componemt were keratin positive and four cases were vimentin positive.Conclusions:RSC is a high grade malignant epithelial tumor with the features of biphasic differentiation.Cinicopothologic stage is a most significant factor that influences the prognosis of the patients.
