2型糖尿病肾病患者尿单核细胞趋化蛋白1和血细胞间黏附分子1与炎症反应的关系

摘    要:目的 探讨糖尿病患者尿单核细胞趋化蛋白1(MCP-1)和血细胞间黏附分子1(ICAM-1)水平与糖尿病肾病的发生发展及炎症反应的关系。方法将90例糖尿病患者按尿白蛋白排泄率(UAER)分为正常白蛋白尿(NA)组、微量白蛋白尿(MA)组、临床白蛋白尿(CA)组,分别测定血、尿MCP-1与血sICAM-1和高敏C反应蛋白(hsC-RP),并与30名健康对照组(NC)比较。结果NC、NA、MA、CA组间的血MCP-1水平差异无统计学意义(P〉0.05),尿MCP-1及血sICAM-1水平逐渐升高,差异有统计学意义(P〈O.05或P〈0.01);尿MCP-1及血sICAM-1水平与UAER均呈正相关(r=0.891,P〈0.01;r=0.583,P〈0.01);尿MCp-1及血sICAM-1水平与血hsC-RP均呈正相关(r=0.723,P〈0.01;r=0.625,P〈0.01)。结论尿MCP-1及血slCAM-1水平与糖尿病肾病炎症反应程度有一定关系,可作为肾脏损害程度及其发生发展的判断指标。

Relationship of urine monocyte chemoattractant protein-1 and serum intercellular adhesion molecule-1 with inflammatory response in diabetic nephropathy of T2DM patients

Abstract:Objective To study the correlation of monocyte chemoattractant protein-1 (MCP-1)and serum intercellular adhesion molecule-1 (sICAM-1) with inflammatory response in diabetic nephropathy of T2DM patients. Method 90 diabetes mellitus patients were divided into three subgroups (n = 30, each) : normo-albuminuria (NA), micro-albuminuria (MA) and clinical albuminuria (CA). The MCP-1, sICAM-1 and hsC-RP were determined by ELISA respectively , which were compared with 30 normal controls (NC). Results The levels of serum MCP-1 did not differ among groups of NC, NA, MA and CA (P〉0.05). The levels of urine MCP-1 and serum sICAM-1 were progressively higher in CA group than in MA(P〈0.05 or P〈0.01), in MA than in NA (P〈0.01), and in NA than in NC (P(0. 01) The urine MCP-1 and serum sICAM-1 correlated positively with UAER (r=0. 891,P〈0. 01 ;r=0. 583, P%0. 01)and with serum hsC-RP (r=0. 723,P〈0.01;r=0. 625,P〈0. 01). Conclusion The urine MCP-1 and serum sICAM-1 are associated with kidney damage and inflammatory response, and might serve as clues to potential predictors for the development of diabetic nephropathy.
