肺上皮样血管肉瘤临床病理观察

目的 探讨肺上皮样血管肉瘤（EA）的病理形态学特点及其鉴别诊断。方法 对1例肺EA的组织病理学、免疫组化结果进行观察分析，结合文献进行讨论。结果 肿瘤有大片的坏死出血区。瘤细胞具上皮细胞的形态学特征，异型性明显，CD31和vimentin（＋）。结论 肺EA具有软组织EA的一般病理形态学特征，但出血、坏死更为显著。CD31是敏感性较高的标记物。主要与上皮样血管内皮瘤、恶性间皮瘤、硬化性血管瘤、肺腺癌相鉴别。

Clinicopathological features of pulmonary pithelioid angiosarcoma

Objective To study the pathological characteristics of the epithelioid angiosarcoma(EA) in the lung and to propose its differential diagnosis.Methods A patient of pulmonary primary EA was analyzed with pathological morphology and immunohistochemical staining and review of the related literatures. Results There was a large hemorrhagic and necrotic zone in the center of tumor.Tumor cells presented epithelioid features with obvious atypia and expressed CD31 and vimentin.Conclusions Pulmonary primary EA has the same features with the EA in soft tissue,but has the more remarkable hemorrhage and necrosis.CD31 is a more sensitive marker.It should be differentiated from the malignant mesothelioma,epithelioid haemangioendothelioma,sclerosing haemangioma and adenocarcinoma of the lung.
