黏软骨膜瓣移位加筛骨垂直板修补鼻中隔穿孔

目的探讨应用鼻内镜下黏软骨（骨）膜辩移位加筛骨垂直板修补中等以上鼻中隔穿孔的效果。方法自一侧鼻腔底部作一黏骨膜瓣以鼻中隔穿孔下缘为蒂经穿孔处翻向对侧，于黏骨膜下嵌入式植入一块略大于穿孔的筛骨垂直板支架封闭穿孔，自另一侧鼻腔顶部作一黏软骨膜瓣以鼻中隔穿孔上缘为蒂经穿孔处翻向对侧，井缝舍固定。结果7例患者鼻中隔穿孔均1次修补成功，随访6—12个月，未再出现穿孔。结论鼻内镜下双侧黏软骨（骨）膜瓣移位加筛骨垂直板是治疗鼻中隔穿孔的理想方法。

Mending Perforation of Nasal Septum by Shifting Mucoperiosteal Flap and Perpendicular Plate of Ethmoid Bone under Nasal Endoscope

Objective To explore the effect of mending the perforation of nasal septum by shifting the mucoperiosteal flap and perpendicular plate of the ethmoid bone under nasal endoscope. Methods The mucoperiosteal flap made of one side of the bottom of nasal cavity was rotated to the other side through the bottom of the perforation of nasal septum and a piece of perpendicular plate of the ethmoid bone was inlayed under the mucoperiosteal flap. Then another mucoperiosteal flap made of the other side of the roof of nasal cavity was rotated to the other side through the roof of the perforation of nasal septum,then tightened and sutured. Results All the 7 cases have cicatrized after once operation. After a following - up period of 6 - 12month, there was no perforation reoccurring. Conclusion Mending the perforation of nasal septum by shifting the mucoperiosteal flap and perpendicular plate of the ethmoid bone under nasal endoscope is an effective method.
