冠心病患者ET和ADM与冠状动脉狭窄关系的探讨

摘    要:目的 探讨冠心病患者血浆内皮素 (ET)和肾上腺髓质素 (ADM )与冠状动脉狭窄程度之间关系。方法 用放免法测定 52例冠心病患者的ET和ADM ,并做冠状动脉造影术 ,证实狭窄程度 ,用目测累积记分法 (CS)表示。研究组 4 2例 ,按含CHD易患因素的多少分 3组。对照组 10例 ,无易患因素、无心绞痛、无心梗病史、冠造无狭窄。结果 ET值和ADM值研究组 (有狭窄者 )明显高于对照组 (P <0 0 5) ;含CHD易患因素越多 ,这种增高越明显 (P <0 0 5) ;52例总的ET值与ADM值相关性良好 (r =0 74 5,P =0 0 0 1)。A、B、C3组CS积分无差异 (P >0 0 5)。结论 有冠脉狭窄者ET和ADM值增高 ,说明存在内皮系统损害 ,并且易患因素增多对这种损害有叠加作用 ,但狭窄程度并未随着ET和ADM值增加 ,说明内皮损害不是冠脉狭窄的唯一决定因素。另外ET与ADM良好的相关性也说明ADM能够反应内皮损害。

Abstract:Objective To discuss the relation between EF and ADM of coronary heart disease (CHD) and coronary artery stenosis. Methods EF and ADM of 52 cases of CHD patient were detected with radio-immunity method and the stenosis of artery was showed with CS method after confirmed with coronary arteriography. 42 cases in test group were divided into 3 groups according to liability factor of CHD. 10 in control group were normal in history and clinic data. Results The EF and ADM in test group were significantly higher than that in control group (p<0.05). More liability factors, the more obviously EF and ADM increased. The total EF value of 52 cases was correlated well with ADM. Conclusion The increased EF and ADM of coronary artery stenosis patient indicated there was endothelial injury that superimposed with the increasing of liability factors. This demonstrated that endothelial injury might not be only one determined factor for coronary artery stenosis. The good relation between EF and ADM showed that ADM could reflect the endothelial injury.
