实时三维超声对法洛四联症的剖析及诊断价值

摘    要:目的探讨实时三维超声心动图(RT-3DE)在法洛四联症诊断中的价值及三维剖析方法。方法以手术结果为标准,对59例法洛四联症患儿运用RT-3DE进行剖析诊断,并与二维超声(2DE)诊断结果比较。结果四腔立体观经剖切后获得的心室-大动脉立体观,瓣膜鸟瞰观及室间隔左、右室面立体观是心脏节段分析较直观的剖视观。以手术结果为标准,RT-3DE与2DE诊断准确性基本一致(P〉0.05)。结论四腔立体观结合肺动脉长轴立体观,可准确诊断法洛四联症。与2DE相比,RT-3DE能提供更为丰富的空间检查信息,具有更高的临床应用价值。

Abstract:Objective To explore the diagnostic values of transthoracic real-time three-dimensional echocardiography (RT-3DE)and 3D topologic methods in the diagnosis of children tetralogy of Fallot (TOF).Methods Fifty-nine children with TOF were diagnosed on RT-3 DE using topologic method.The results were compared with two-dimensional echocardiography (2 DE)and surgical operations(golden standard).Results Ventricular-arteries,upward valvular view,right side and left side of interventricular septum(IVS)3D views could be obtained through topologic four-chamber 3D views,which were intuitive views for heart segmental analysis.Compared with surgical operations,the accuracy of RT-3DE and 2DE were closely consistent(P>0.05).Conclusions TOF can be accurately diagnosed through four-chamber 3D view combined with pulmonary artery longitudinal axis 3D veiw.RT-3DE with topologic method can give more special informations and it is more valuable in the diagnosis of TOF than that of 2DE.
