探讨腰-硬联合麻醉应用于老年下腹部手术的效果

目的 对腰-硬联合麻醉应用于老年下腹部手术的临床效果展开分析.方法 随机选取2010年5月-2012年5月期间我院收治的90例接受下腹部手术的老年患者,分成观察组和对照组,观察组45例,采用左步比卡因腰-硬联合麻醉;对照组45例,采用硬膜外麻醉.对比两组患者围手术期间的心率、血压、氧饱和度指标的变化情况,麻醉效果及副作用.结果 两组患者接受麻醉后,0.5h时,两组患者HR、SPO2、SBP、DBP差异不明显,差异都不具备统计学意义（P＞0.05）.对比两组患者接受对应麻醉应用的效果,观察组显著优于对照组,差异具有统计学意义（P＜0.05）;同时观察组阻滞完善性、麻醉副作用效果显著优于对照组,差异具有统计学意义（P＜0.05）.结论：腰-硬联合麻醉应用于老年下腹部手术,具有快速、镇痛、维持患者心血管稳定、降低麻醉副作用的功效.

Discussion on combined spinal-epidural anesthesia in elderly abdominal operation effect

Objective To analyze the clinical effect of combined spinal-epidural anesthesia in elderly abdominal operation. Methods Randomly selected from 2010 May -2012 year in May in our hospital 90 cases accepted abdominal operation in elderly patients, divided into the observation group and the control group, the observation group 45 cases, the left step bupivacaine in combined spinal-epidural anesthesia; 45 cases in the control group, the epidural anesthesia. The changes were compared between two groups during operation period of heart rate, blood pressure, oxygen saturation index, anesthesia effect and side effects. Results The two groups of patients after anesthesia, 0.5H, HR of two groups of patients, SPO2, SBP, DBP were not significant, difference does not have statistical significance （P 〉 0.05）. The anesthesia effect of two groups of patients received contrast, the observation group was significantly better than the control group, the difference was statistically significant （P 〈 0.05）; at the same time, the observation group complete block, anesthesia effect was better than the control group, the difference was statistically significant （P 〈 0.05）.Conclusion The combined spinal-epidural anesthesia in elderly abdominal operation, has the advantages of fast, analgesia, maintaining stability, reducing anesthesia for patients with cardiovascular side effect.
