多层螺旋CT三维重建对不同部位儿童气管支气管异物的诊断价值

目的 探讨多层螺旋CT三维重建对不同部位儿童气管支气管异物的诊断价值.方法 回顾性分析湖南省儿童医院2011年9月至2013年2月经纤维支气管镜检查并确诊为儿童气管支气管异物的患儿114例,所有患儿检查前均行胸部多层螺旋CT三维重建.结果 气管异物病例的CT阳性率最低（7.70%）,其次为叶及叶以下支气管异物病例,CT阳性率为58.33%,右主支气管异物、左主支气管异物病例的CT阳性率相对较高,分别为81.82%、74.19%,差异有统计学意义（χ2=34.82,P〈0.0001）.结论 多层螺旋CT三维重建对不同部位儿童气管、支气管异物的诊断价值存在差异,气管异物CT阳性率低,易漏诊、误诊,需尽早行支气管镜检查确诊.

Evaluation of multi-slice spiral CT three- dimensional reconstruction in diagnosis of tracheobronchial foreign body in different parts of the bronchus

Objective To evaluate the value of multi-slice spiral CT three-dimensional reconstruction in the diagnosis of tracheo- bronchial foreign body in different parts of the bronchus. Methods From 2011 September tO 2013 February,114pediatric racheo- bronchial foreign body cases confirmed by fiberoptic bronchoscopy were enrolled in this study. All of the patients before the exam- ination underwent multi-slice spiral CT three-dimensional reconstruction. Results The positive rate of CT tracheal foreign body were the lowest （7.70%）, followed by the lobar bronchus foreign body cases, the positive rate of CT was 58.33%,the positive rate of CT in right main bronchus and left main bronchus foreign body was relatively high, respectively 81.82%,74.19%. And the differ- ence Was significan （X^2=34.82,P〈0.0001）t. Conclusion Diagnostic value of multi-slice spiral CT three-dimensional reconstruc- tion in dfferent parts of the trachea,bronchus foreign body in children are differences. Especially in cases of suspected foreign bod- ies in trachea,negative CT should also take into account tracheal foreign body.
