HBV血清标志物与HBV-LP及HBV-DNA联合检测的临床意义初探

目的:探讨乙型肝炎病毒(HBV)感染的不同模式与乙型肝炎病毒外膜大蛋白(HBV-LP)及乙型肝炎病毒DNA(HBV-DNA)载量之间的相互关系,为临床上乙型肝炎的诊断及治疗提供重要的理论指导与参考依据。方法:对1210例临床采集的血清标本同时检测其乙肝标志物、HBV-LP以及HBV-DNA载量。结果:I(大三阳)模式组,即HBsAg、HBeAg、HBcAb阳性的患者,HBV-LP阳性率显著高于其余各个血清模式组(P<0.05);同时I(大三阳)模式组HBV-DNA阳性率也高于其余各个血清模式组(P<0.05)。I(大三阳)组的HBV-DNA载量显著高于II(小三阳)组(P<0.05);II(小三阳)模式组与III(HBsAg、HBcAb阳性)模式组相比,HBV-DNA水平无显著性差异(P>0.05)。不同感染模式中,血清HBV-LP与HBV-DNA阳性率差异无统计学意义(P>0.05)。结论:HBV-LP能较准确地反映HBV复制水平,是监测HBV感染者体内病毒复制疾病进展与疗效的较好指标;HBV血清标志物、HBV-LP和HBV-DNA载量三者联合检测有利于全面监测乙肝病情和提高乙肝疗效。

Study on Co-detection of HBV Serum Immunological Markers,HBV-LP and HBV DNA

Objective To investigate the correlationamong HBV infectio nmodels,HBV-DNA loads and HBV-LP in serum and to provide evidence for the diagnosis and treatment of hepatitis B.Methods Hepatitis Bmarkers,HBV-LP andHBV-DNA loads in serum samples were detected simultaneously for 1210 cases.Results Thepositive rate of HBV-LP was higher in HBsAg,HBeAg and HbcAb positive model than that in other models(P<0.05).Besides,the positive rate of HBV-DNA was higher in HBsAg,HBeAg and HbcAb positive model than that in other models(P<0.05).Furthermore,the HBV load was higher in HBsAg,HbeAg and HbcAb positive model than that in HBsAg,HbeAb and HbcAb positive model(P< 0.05).The results of HBV load showed no significant differences between HBsAg,HbeAb and HbcAb positive model and HBsAg,HbcAb positive model(P>0.05).Conclusion Serum HBV-LP is laboratory marker that can accurately reflect HBV-DNA reproduction,is valuable for monitoring HBV-DNA replication therapeutic effect and progression of the disease.The combination assay of serum HBV markers,HBV-LP and HBV-DNA load was beneficial to the monitoring of disease condition of hepatitis B and to the improvement of the rapeutic effect.
