氩氦刀冷冻兔肝后局部组织变化及超微病理研究

摘    要:目的 探讨氩氦刀冷冻时间与肝组织效应,为临床上肝脏肿瘤氩氦刀治疗提供实验依据.方法 用直径2mm氩氦刀置于兔肝一叶表面,分别以5、10min冷冻肝组织后,在1、2、4、8、16天取肝组织,在光、电镜下观察肝脏的组织与超微病理变化.结果 冷冻后第1天,肝组织明显地分为中央区、非炎性渗出带、出血带、交界线.第2天,非炎性渗出带呈断续状.第4天,非炎性渗出带消失,交界线上出现纤维细胞增生带.第8天,交界处纤维组织增生带增宽.第16天,该带进一步增宽,带中出现了多核巨细胞.冷冻的肝组织区均呈坏死状,在全部实验动物中未见肝组织复活的现象.结论 氩氦刀冷冻5min、10min均破坏了冷冻区内的肝组织,在出现非炎性反应后,损伤周围出现纤维组织增生,形成结缔组织增生带包裹损伤区.

Abstract:Objective To explore the effects of different cryotherapy time by cryocare TM on histology and ultrastructure of hepatic tissue in rabbits. Methods The cryoprobe 2mm in diameter was set on the surfaces of levers in 50 tabbits. One group was frozen for 5 minutes and another 10 minutes. The hepatic tissues which were taken respectively on 1, 2, 4, 8, 16 days after the cryotherapy were observed under light and electron microscope. Results The hepatic tissue was obviously divided into central area, non-inflammatory infiltraltion zone, hemorrhagic zone, boundary layer on the first day after freezing. The non-inflammatory in filtration zone was discontinuous on the second day after freezing and disappeared on the fourth day after freezing, when the zone of fibroblast proliferation appeared in the boundary zone. The the width of the zone of fibroblast proliferation increased gradually on the eighth and sixteenth day after freeing. The degree of damage to lever frozen by ten minutes was more serious than that by five minutes at the earlier stage, but no recovery of hepatocyte was found from beginning to end in both the group. Conclusions The hepatic tissue in freeing area is completely destroyed after crytherapy for 5 and 10 minutes, and will be wrapped gradually by conneetive tissue.
