| 摘 要： | 目的探讨腰-硬联合麻醉在剖宫产术中的应用。方法将120例临产妇随机均分为CSEA和EA两组,CSEA组采用腰-硬联合麻醉包穿刺,经L_(2-3)间隙硬膜外穿刺成功后,以针内针腰穿,见脑脊液后缓慢注入重比重布比卡因8～10mg(0.75%布比卡因+10%葡萄糖1ml),然后向头端置入导管3～4cm,转为仰卧位后,调节平面达T_6。EA组在相同间隙穿刺置管,注入2%利多卡因5ml试验量后,分次追加局麻药(1.6%利多卡因)至阻滞完善平面达T6水平。术中监测BP、SpO_2、ECG,记录I-D、手术时间及新生儿1min和5min的Apgar评分。术后24h随访并记录VAS及并发症的发生率。结果两组新生儿1min和5min的Apgar评分均在8分以上;CSEA组I-D时间明显比EA组短;麻醉效果CSEA组明显优于EA组;CSEA组低血压的发生率高于EA组。结论腰-硬联合麻醉结合了EA和SA的优点,在产科手术逐渐被广泛使用,麻醉效果好。
