尿激酶联合自体血浆治疗实验性兔脑室出血

摘    要:目的 研究尿激酶和尿激酶联合自体血浆溶解兔脑室内血凝块的效果。方法 成年兔40只,每只体重3 kg左右,向兔侧脑室内注射0.2ml兔自体血浆,建立兔脑室出血模型。用随机方法分为3组:等渗盐水组(10只),尿激酶组(15只),尿激酶+自体血浆组(15只)。3组分别接受脑室内注入等渗盐水0.15 ml;尿激酶0.1 ml+等渗盐水0.05 ml;尿激酶0.1 ml+自体血浆0.05 ml,尿激酶的浓度为5万U/ml。冠状位切片观察脑室内血块溶解情况。 结果 术后3 d等渗盐水组9只兔,有1只兔血块溶解(11%);尿激酶组13只兔,9只兔血块溶解(69%P=0.011);尿激酶+自体血浆组12只兔,10只兔血块溶解(83%),与尿激酶组比较,P>0.05。有5只兔制造模型不成功,1只兔经制造模型和治疗后死亡,故这6只兔未列入统计。 结论 尿激酶能显著促进兔脑室内血凝块的溶解,自体血浆辅助尿激酶有增强溶解血凝块的作用。

Abstract:Objective To study the efficacy of urokinase(UK) combined with autologous plasma on the lysis of intraventrieular blood clots in the rabbit intraventricular hemorrhage model. Methods Intraventricular hemorrhage was induced by intraventricular injection of 0. 2 ml clotted autologous blood. The rabbits were divided randomly into 3 groups, through the intraventricular cannula the 3 group of rabbits were received 0. 15 ml of isotonic saline (IS), 0. 1 ml of UK + 0. 05 ml of IS, and 0. 1 ml of UK + 0. 05 ml of autologous plasma respectively. The concentration of UK was 50 000 U/ml. Lysis of blood was observed on brain coronal sections. Results Clot lysis had been achieved in nine (69% ) of 13 UK-treated animals as compared to one (11% ) of 9 IS-treated animals ( P = 0. 011). Clots had been lysed in ten (83% ) of 12 UK + plasma-treated animals. There was no statistical significance when this group was compared with the UK-treated group. Conclusion UK markedly enhances the lysis of blood clots in ventricles of rabbits. Compared to UK administered alone, intraventricular administration of autologous plasma combined with UK showed an increased action in the lysis of intraventricular blood clots.
