BiPAP治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭疗效观察

目的探讨BiPAP治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床疗效。方法选择2006年1月-2009年6年收治于我院的128例慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者，随机分为两组，对照组采用常规治疗，观察组在对照组的基础上加用BiPAP治疗，治疗后，比较两组患者的治疗前、治疗后的pH值、动脉Pa02、PaCO2变化情况。结果通过治疗，两组患者的血气指标均达到一定的改善，患者的PaO2上升、PaCO2下降。治疗后观察组血气指标也优于对照组，差异均有显著性（P〈0．05）。结论BiPAP治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭安全、有效的方法，值得临床推广应用。

The effect of COPD with tapeⅡrespiratory failure by BiPAP

Objective To evaluate the clinical effect of by BiPAP for patients with COPD and tape D respiratory failure. Methods Choice 128 cases with hypoxemia by COPD in our hospital from 2006.1 to 2009.6. were divided into BiPAP group and conventional therapy treatment group, the BiPAP group were treated by conventional therapy at the same time to use BiPAP, the conventional therapy treatment group were only treated by conventional therapy.After therapy, 128 cases' PaO2 was ascensus, PaCO2 were decreased. There were significance level of pH, PaO2 and PaCO2 between (P< 0.05).Results After therapy, 28 cases'RR and HR were decreased, PaO2 and SaO2 were ascensus, PaCO2 were decreased. There were significance level of RR, HR, PaO2, SaO2 and PaCO2 between observation groups and control groups(P< 0.05). Conclusion COPD with tape II respiratory failure treated by BiPAP is very safity and efficacy, it's worth to clinical application.
