椎管内用药救治急性外伤性脊髓损伤

目的观察外伤性脊髓损伤急性期的治疗效果。方法将48例急性脊髓损伤病人在接诊数小时内即给以椎管内注射地塞米松与回苏灵，每日1次，连续5～7次；同时酌情给以静脉滴注甘露醇、地塞米松、抗生素等药物以及对症处理。结果治疗1周后Ⅰ级以上恢复者29例，占60．4％；4周后Ⅰ级以上恢复者43例，占89．6％。椎管内注射上述药物，无明显副作用。结论椎管内注射地塞米松与回苏灵，能有效减轻脊髓水肿与外伤后迟发反应，对病人的呼吸肌与自主神经功能有促进作用，促进神经功能恢复，缩短病程，为其他综合治疗创造条件与赢得时间。

TREATMENT OF ACUTE INJURIES OF SPINAL CORD WITH INTRASPINAL THERAPY

AIM To observe the treatment effect of intraspinal therapy for acute spinal cord injuries. Method 48 patients with spinal cord injuries received injection of dexamethasone and dimeflinum in vertebral canal with subordinate intravenous drip of mannital,dexamethasone and other antibiotics. Result 1 week after the treatment 29 cases turned out effective (60. 4% ), and 4 weeks after the treatment 43 patients showed effect (89.6% )and no side effect was found. Conclusion Dexamethasone and dimeflinum injected in vertebal canal can relieve hydrops of spinal cord and delay reaction after injury and can improve the patients' autonomic nerve function as well as the function of respiratory muscle.
