高产AmpC酶阴沟肠杆菌的耐药性及同源性分析

摘    要:目的掌握阴沟肠杆菌的耐药谱及分子流行病学特征.方法高产AmpC酶采用表型筛选法,耐药谱选用改良K-B法,基因分型采用随机扩增多态DNA(RAPD)法.结果50株阴沟肠杆菌中表现高产AmpC酶株有16株(32%)、单产ESBLs菌株有8株(16%)、同时产AmpC酶和ESBLs的菌株有8株(16%),未发现亚胺培南耐药株,产酶和不产酶菌株间的耐药率差异存在显著性,RAPD基因分型得的遗传聚类图,可分为4类.结论阴沟肠杆菌有很高的耐药性,阴沟肠杆菌感染的治疗应首选亚胺培南,RAPD技术对阴沟肠杆菌感染同源性分析是一个有效的方法.

Abstract:OBJECTIVE To investigate the epidemiological characteristics and drug resistance of Enterobacter cloacae. METHODS Phenotype screening was used for detecting AmpC.Modified K-B was adopted to get drug resistance.RAPD was carried out for genotype. RESULTS Among 50 strains of E.cloacae,16 strains(32%) produced AmpC enzymes highly,8 strains(16%) produced ESBLs and 8 strains(16%) produced both of them.All strains were susceptible to imipenem.Enzyme producing strains had higher resistance rate than non-enzyme producing ones.The genotype was divided into 4 clusters by RAPD. CONCLUSIONS Drug resistance is very serious in our hospital.Imipenem is recommended to deal with E.cloacae infection in empiric therapy.RAPD is the effective method to analyze the homogeneity of E.cloacae.
