腹横筋膜阻滞在全腔镜胃癌根治术中的镇痛作用

目的:了解腹横筋膜阻滞在全腔镜胃癌根治术中的作用。方法:选取西安交通大学第一附属医院普通外科45例行全腹腔镜胃癌根治术患者，随机分为腹横筋膜阻滞组（研究组）23例和对照组22例。记录患者一般情况，术中情况和术后病理分期，观察术后恢复情况和术后并发症发生情况。并连续记录术后不同时间点的视觉模拟评分（VAS）。结果:研究组和对照组在性别、年龄、肿瘤分期、术后并发症、下床时间、排气时间和术后48、60以及72 h的VAS评分上比较无统计学差异。研究组在12 h（1.57±0.99 vs 2.82±1.18）、24 h（1.83±0.72 vs 2.55±0.91）、36 h（1.82±0.48 vs 2.68±0.84）的VAS评分明显优于对照组。结论:腹横筋膜阻滞在全腹腔镜胃癌根治术中具有较好的镇痛作用。

The analgesia of abdominal transverse fascia block in total endoscopic radical gastrectomy

Objective:To investigate the effect of abdominal transverse fascia block on total endoscopic radical gastrectomy for gastric cancer patients.Methods:A total of 45 patients undergoing total laparoscopic radical gastrectomy were selected from General Surgery Department of the First Affiliated Hospital of Xi'an Jiaotong University.They were randomly divided into two groups:Abdominal transverse fascia block group(study group) and control group.The general situation of the patients,the perioperative situation and the postoperative pathological staging and postoperative complications were recorded.The visual analogue score(VAS) was recorded at different time points after the operation.Results:There was no significant difference in gender,age,tumor stage,postoperative complications,ambulation time,the first time of exhaust time and 48,60 and 72 h VAS score between two groups.In study group,12 h（1.57±0.99 vs 2.82±1.18）,24 h（1.83±0.72 vs 2.55±0.91）,36 h（1.82±0.48 vs 2.68±0.84） VAS score were significantly lower than those in control group.Conclusion:Abdominal transverse fascia block has good analgesic effect in total laparoscopic radical gastrectomy for gastric cancer patients.
