妊娠合并糖尿病患者药学监护流程的建立与实践

目的:为妇产科临床药师开展药学监护工作提供参考。方法:在日常药学服务实践工作基础上,针对妊娠合并糖尿病患者治疗过程中常见用药相关问题,建立药学监护流程并实施。结果:依据药学监护流程,临床药师能够及时发现药物相关不良反应、改善患者治疗依从性、对用药错误行为进行干预。结论:建立并实施药学监护流程,有利于临床药师深入开展药学服务,加强与临床医务工作者的协作,保障患者用药的安全性和合理性。

Establishment and practice of pharmaceutical care process for pregnancy complicated with diabetes mellitus

OBJECTIVE To provide a reference for clinical pharmacists in obstetrics and gynecology department.METHODS Based on the clinical pharmacist daily work practice, a pharmaceutical care process was established to solve the common problems among patients suffering diabetes mellitus during pregnancy.RESULTS According to the process, clinical pharmacists could timely detect adverse drug reactions, improve patient treatment compliance, and intervene with the medication errors.CONCLUSION The pharmaceutical care process is beneficial to clinical pharmacists to carry out pharmaceutical service in depth, strengthen the cooperation with other medical workers, and guarantee the safety and rational use of medicines.
