标准桃金娘油联合糜蛋白酶治疗放射性鼻窦炎随机对照研究

目的:探讨标准桃金娘油联合糜蛋白酶鼻腔冲洗治疗放射性鼻窦炎的有效性。方法:对于放射性鼻窦炎的分级采用3种评分方法，包含症状量表评分法（VAS）、内镜评分法（Lund-Kennedy）和CT评分（Lund-Mackey），在放疗前、放疗后12个月对放射性鼻窦炎的情况进行评分。通过随机对照实验验证干预措施的有效性。随机分组采用数字表法进行。纳入研究的鼻腔鼻窦肿瘤患者共60例。标准桃金娘油联合糜蛋白酶鼻腔冲洗的患者进入实验组，单纯生理盐水进行鼻腔冲洗的进入对照组。结果:标准桃金娘油联合糜蛋白酶组放疗后各项评分较放疗前升高不明显（P＞0.05）。说明实验组对放射性鼻窦炎的预防是有效的。而单纯生理盐水冲洗组治疗后的各项评分均较治疗前明显升高（P＜0.05），即使用单纯生理盐水进行鼻腔冲洗，不能有效预防放射性鼻窦炎的发生。结论:标准桃金娘油口服联合糜蛋白酶鼻腔冲洗较单纯生理盐水鼻腔冲洗，更能够有效预防放射性鼻窦炎的发生。

A randomized controlled study of standard myrtle oil combined with chymosin to treat radioactive sinusitis

Objective:To explore the efficacy of standard myrtle oil capsule combined with chymotrypsin for the treatment of radioactive sinusitis.Methods:The radioactive sinusitis was diagnostically determined by subjective and objective evaluation methods,visual analogue scale (VAS),nasal endoscopy by Lund-Kennedy scoring system,and Lund-Mackay CT scoring system before treatment and twelve months after the treatment.A total of 60 patients with nasal sinus tumors were enrolled by random digital table method.The prospective randomized controlled study was designed by groups of standard myrtle oil capsules and chymotrypsin in experimental group,nasal wash with physiological saline in control group.Results:Randomized controlled study showed that,the VAS score,Lund-Kennedy and Lund-Mackey score increased significantly in the patients after therapy in the control group.In other word,the incidence of sinusitis increased after radiation compared with pre-radiation in this control group (P＜0.05).But in the experimental group,these scores did not increase significantly in the patients after radiotherapy (P＞0.05).Data thus indicated that application of intervention was valid for the prevention of radioactive sinusitis.Conclusion:It was effective to treat RNS with myrtol standardized enteric capsules and chymotrypsin confirmed by randomized controlled trial (RCT).
