扩大性部分膀胱切除治疗脐尿管癌（附3例）

目的:评价腹腔镜扩大性膀胱部分切除术治疗脐尿管癌的安全性和有效性。方法:回顾分析我院3例脐尿管癌患者的临床资料并文献复习,均采用腹腔镜扩大性膀胱部分切除术。结果:本组3例手术均成功，无中转开放。手术平均时间120(80~190) min；术中平均出血95(70~120) ml；术后平均住院4（3.5~4.5）天；术中、术后无明显并发症，术中、术后病理均证实术前诊断，病理切缘阴性；平均随访22.5（16~36）个月，放射学或膀胱镜检查未发现复发。结论:脐尿管癌患者行腹腔镜下扩大性膀胱部分切除术是安全、可行、疗效确切的方法，具有微创、手术视野清晰、恢复快、住院时间短等优点，对于仔细筛选的患者是值得推广的一种微创方法。

Extended partial cystectomy for treatment of urachal carcinoma(3 cases)

Objective:To evaluate safety and effectiveness of laparoscopic extended partial cystectomy for urachal carcinoma.Methods:Combined with the three cases of laparoscopic extended partial cystectomy for urachal carcinoma of our hospital and review of the literature reported.Results:All three procedures were completed laparoscopically without open conversion.The average operating time were 120(80~190) minutes,with a average estimated blood loss of 95(70~120) ml,the average hospital stay were 4(3.5~4.5) days.There were no intraoperative or postoperative complications.Frozen section evaluations of bladder margins were routinely obtained and were negative for cancer in all cases.Final histopathology confirmed urachal adenocarcinoma in 3 cases.At an average follow-up of 22.5(16~36)months,there was no evidence of recurrent disease as evidenced by radiologic or cystoscopic evaluation.Conclusion:Laparoscopic extended partial cystectomy in carefully selected patients with urachal carcinoma was feasible and safe and has the advantages of little trauma,clear surgery vision,faster postoperative recovery,offering a promising and minimally invasive alternative for these patients.
