前房注射莫西沙星预防白内障术后眼内炎的Meta分析

目的：评价前房注射莫西沙星预防白内障术后眼内炎的有效性。方法：计算机检索PubMed、EMbase、The Cochrane Library、Clinical Trial数据库，搜集关于前房注射莫西沙星预防白内障术后眼内炎的研究，检索时限为建库至2017年7月。由2位研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后，采用Stata 12.0软件进行Meta分析。结果：共纳入8项研究，Meta结果显示，围手术期前房注射莫西沙星可以减少白内障术后眼内炎的发生率OR=0.26，95% CI （0.20，0.34），P=0.000]。敏感性分析提示，剔除权重较大的研究重新进行分析，发现结果相似。采用Begg's和Egger's检验纳入研究的发表偏倚，提示纳入研究存在发表偏倚的可能性小。结论：前房注射莫西沙星可以有效减少白内障术后眼内炎的发生率，但该措施在临床上普遍推广使用仍值得商榷。

Anterior chamber injection of moxifloxacin in prevention of endophthalmitis after cataract surgery: a meta analysis

OBJECTIVE To evaluate the efficacy of moxifloxacin in the prevention of intraocular gastritis after cataract surgery.METHODS Database such as PubMed, EMbase, The Cochrane Library and Clinical Trials were searched for studies on anterior chamber injection of moxifloxacin in the prevention of intraocular gastritis after cataract surgery till July 2017. Meta-analysis was performed using Stata 12.0 software after two researchers independently screened the literature, extracted the data and evaluated the bias risk of inclusion studies.RESULTS A total of eight studies were included. Meta results showed that moxifloxacin in the anterior chamber could reduce the incidence of endophthalmitis after cataract surgeryOR=0.26, 95% CI (0.20, 0.34), P=0.000]. Sensitivity analysis suggested that, after studies with the larger weight rejected, reanalysis showed similar results. The use of Begg's and Egger's tests included the published bias in the study, suggesting that the inclusion of the publication bias was unlikely.CONCLUSION The injection of moxifloxacin in the anterior chamber can effectively reduce the incidence of endophthalmitis after cataract surgery, but it is still debatable in clinical practice.
