综合心理治疗对骨折术后患者睡眠质量的影响

目的探讨综合心理治疗对骨折术后患者睡眠质量及心理状态的影响。方法选择2012年1月-2013年12月在我市骨科医院住院的骨折术后患者76例,随机分为对照组32例和治疗组44例,对照组采用常规骨科治疗与护理;治疗组在常规治疗的基础上给予综合性心理治疗。对两组患者治疗前后进行匹兹堡睡眠质量指数(PSQI)及焦虑自评(SAS)和抑郁自评(SDS)[1]的评分比较分析。结果 1心理治疗前两组患者PSQI总分和各成分得分差异无显著性(t=0.108~0.343,P0.05),两组间SAS,SDS评分差异无显著性(t=0.378,0.592;P0.05);2综合性心理治疗后治疗组患者的PSQI总分,睡眠质量、入睡时间,睡眠时间、睡眠效率、睡眠障碍得分显著低于对照组(t=3.46~6.00,P0.01);日常功能障碍(t=2.201,P0.05)治疗组的SAS,SDS评分明显低于对照组(t=15.16,10.580;P0.01)。结论综合性心理治疗可以有效地改善骨折术后患者的睡眠质量及缓解其负性情绪。

Effect of Comprehensive Psychotherapy on Sleep Quality of Patients after Fracture Operation

Objective To explore the effects of comprehensive psychotherapy on sleep quality and mental state of patients with fracture. Methods 76 cases,hospitalized in 2012 January- 2013 December in department of orthopedics fracture after operation were randomly divided into two groups,32 cases in the control group and the treatment group 44 cases,control group with conventional therapy and nursing in department of orthopedics,the treatment group was given comprehensive psychotherapy on the basis of conventional therapy. Pittsburgh sleep quality index（ PSQI）,self rating anxiety（ SAS） and self rating depression（ SDS） were used to assess. Results 1 There were no significant differences in PSQI,SAS and SDS scores between two groups before treatment. 2 Patients in the treatment group had lower scores in PSQI,sleep quality,sleep time,sleep efficiency,sleep disorder and daily dysfunction than those in the control group（ t= 2. 201 ~ 6. 00; P 0. 05） after comprehensive psychological treatment; Their SAS and SDS scores were significantly lower（ t = 15. 16,10. 580; P 0. 01）. Conclusion Comprehensive psychological therapy can effectively improve sleep quality of patients after operation and alleviate the negative emotional fracture.
