摘    要:目的 了解补体系统在人类急性缺血性脑血管疾病中的激活情况及其作用。方法 分别于脑卒中后4h、12h、2 4h、4 8h、72h、7d、14d、30d抽血,检测新发脑卒中病人血液中补体C3 的表达情况及C反应蛋白(CRP)的表达情况。相同的血液标本从对照组获得。结果 对照组和病例组CRP的表达在早期不存在显著性差异(P >0 . 0 5 ) ,CRP于卒中后2 4h显著增加,至4 8h仍增加明显,72h逐渐降低,至7d仍保持较高水平,30d基本恢复正常。C3 的浓度在卒中早期也不存在显著性差异(P >0 . 0 5 ) ,从72h开始显著增加,7d达高峰,14d已有降低,以后逐渐恢复正常。结论 急性缺血性脑血管疾病早期存在短暂的补体激活。

Abstract:Objective There is growing evidence that compleme nt is involved in the process of pathophysiological damage of ischemic stroke, whi ch is one of the major systems of inflammation. We investigate the expres sion and the role of complement in human ischemic stroke. Method Plasma samples were obtained at the following time-points after stroke:4h,12h,2 4h, 48h,72h,7d,14d and 30d, the plasma concentration of complement C3 and CRP was de tected . Samples from controls were obtained at the same time-period as the patient's did. Results There was no sign ificant difference in CRP concentration between stroke patients at the stroke on set and controls (P>0.05), CRP increased significantly at 24h, remained s ignificantly increased at 48h, decreased gradually at 72h, remained at higher le vel at 7d, recover to normal at 30d. There was no difference in C3 concentration between stroke patients at the stroke onset and controls (P>0.05), they i ncreased significantly at 72h, reached peak at 7d, it has decreased at 14d ,then recovered to normal gradually. Conclusions Transitory systemic complement activation takes place after acute ischemic stroke.
