气囊助产在头位难产中的临床应用价值

摘    要:目的:探讨气囊助产在头位难产中的临床应用价值。方法:将600例头位难产者随机分成实验组300例和对照组300例,实验组应用气囊助产后再徒手旋转胎头;对照组只徒手旋转胎头。结果:实验组自然分娩208例(69.3%),阴道手术产20例(6.7%),总的阴道分娩率76%,剖宫产72例(24%),产程较短,活跃期平均(3±0.5)h,第二产程平均(30±10)min;对照组自然分娩18例(6%),阴道手术助产15例(5%),总的阴道分娩率11%,剖宫产267例(89%),产程活跃期平均(6.5h±0.3)h,第二产程平均(2±0.5)h,两组比较,差异有统计学意义(P<0.01)。结论:头位难产经气囊助产加徒手旋转胎头位置,使多数头位难产变为顺产,降低剖宫产率,且产程较对照组明显缩短。

Abstract:Objective:To explore the value of air sac obstetric apparatus in cephalic dystocia.Methods:Totally 600 cephalic dystocia cases were randomly divided into two groups: 300 cases in experimental group (using air sac obstetric apparatus and then circumgyrate foetus' head bare-handedly) and 300 cases in control group (using only bare-handed methods).Results:In experimental groups: normal delivery rate was 208 (69.3%), vaginal operation labor rate was 20 (6.7%), the rate of vaginal labor was 76%, and cesarean section rate 72 (24%);The average duration of active phase in the first stage was 3 h±30 min, the second stage 30 min±10 min; in control groups: normal delivery rate was 18 (6%), vaginal operation labor rate 15 (5%),the rate of vaginal labor was 11%, cesarean section rate 267 (89%).The average duration of active phase in the first stage was 6.5 h±20 min, the second stage 2 h±30 min, there was significant difference compared in these two groups (P<0.01).Conclusion:Using air sac obstetric apparatus and then circumgyrate foetus' head bare-handedly can turn most of dystocia presentation to normal delivery, thus greatly reduce the cersarean section rate and the duration of the labor stage.
