阴茎海绵体压力测定的方法学实验研究

摘    要:为建立阴茎海绵体压力测定(CM)方法,并探讨其测定条件及影响因素,对9例主诉阳萎的志愿者进行CM及药物性化学假体压力测定(PCM),分别采用不同诱导灌注流率(IF)、不同剂量血管扩张剂(罂粟碱)及不同海绵体压力(ICP)设定进行CM及PCM。比较CM及PCM在上述不同测定条件下所得参数:IF、维持灌流率(MF)及压力跌差(PLC)。结果表明:CM时快速与慢速灌注测得的MF差异显著,而PCM时无显著差异;罂粟碱为30及60mg时测得的MF差异显著,ICP设定低于系统舒张压(7.84kPa)时PCM测得的MF显著低于11.76kPa与14.7kPa时;PLC测定值较为稳定。结论:有IF、海绵体平滑肌扩张程度、ICP设定值等多种因素影响CM,诊断静脉性阳萎时应采用PCM,血管扩张剂用量应充分,ICP设定应高于系统舒张压,使得PCM测定条件尽量恒定,减少影响测定的因素。

Abstract:In order to set up the method of cavernosometry (CM), to discuss the conditions tested and the factors influencing the test, the cavernosometry and pharmacocavernosometry (PCM) were performed in 9 volunteers with impotence. We evaluated various techniques for CM: with and without intracavernous papaverine, with different dosages, with different modes of induction flow (IF) and at various intracavernous pressure (ICP). The IF, maintenance flow (MF) and pressure loss change (PLC) from CM and PCM at different conditions were compared. The results showed that MF was higher at the fast perfusion than at the slow one in CM; MF was lower at 60 mg papaverine than at 30 mg dosage; MF was lower at 7.84 kPa ICP than at 11.76 and 14.7kPa ICP in PCM; and the differences of PLC from various techniques were not significent. CM and PCM were influenced by IF, dosages, ICP, and other factors, PCM was in highly value with regard to MF for the patients with venous impotence. In PCM, the dosage of papaverine should be enough, ICP must be set up to the pressure higher than dystolic blood pressure, so that the influence factors of CM and PCM will be decreased, and the methodology and the conditions tested will be stable.
