39例变态反应性支气管肺曲菌病的群发

报告变态反应性支气管肺曲菌病39例。三日内群发（某砖瓦厂工人），均有哮喘症状。10例X线胸片，2例浸润，8例轻度间质改变，痰涂片6例见大量真菌菌丝体及孢子，其中1例鉴定为曲霉 ，5例痰培养生长霉菌。经调查患者均食用了在阴暗，潮湿贮存的甘蔗，取该甘蔗培养亦生长霉菌。致病的原因为吸入了污染的甘蔗霉菌，患者均治愈。

A Break Out of Asthma in 39 Workers

An epidemic episode of allergic bronchopulmonary aspergillosis in thirty nine brick workers (M: 36,E: 3, 18-38 yrs)who were previously healthy in Nanchang suburbs has been reported. Before the episode the workers lived in moist houses where lots of fresh sugarcanes were piled. Symptoms of asth- ma broke out within 3 days after the patients had eaten fresh sugarcane. All cases had peroxysmal Cough and 10 cases had low grade fever. Blood eosinophil count increased to more than 1000/mm~3 in 23 cases. Fungal hyphae and spores were found in sputum smear of 6 cases. Fungus grew in cultures of all 5 sputum samples,being identified as aspergillus flavus and penicillium nototum. In the culture of sugarcane also grew aspergillus. Chest film showed interstitial changes of the lung in 8 of 10 cases and spots or plaques of the middle and lower lobes in 2 cases. 9 of the 10 cases undergoing lung function test showed small airway abnormalities. 23 cases were admitted into the hespital. Symptoms disap- peared immediately after receiving aminophylline and prednisone. In 13 of 16 untreated cases symp- toms disappeared gradually. Cough and wheezeg lasted a few days in the other 3. The diagnosis is con- sidered to be allergic bronchopulmonary aspergillosis caused by aspergillus inhalation.
