摘    要:目的了解心脏外科重症监护病房(ICU)获得性感染的病原学及耐药状况.方法对1999年9月至2004年6月心脏手术后入ICU的患者分离的菌株进行回顾性总结,分析其耐药性.结果共分离607株菌株.革兰阴性菌占49.8%,真菌占27.8%,革兰阳性菌占22.4%.在细菌感染中.鲍曼不动杆菌、铜绿假单胞菌、金黄色葡萄球菌、肺炎克雷伯菌、阴沟肠杆菌、溶血葡萄球菌为主要的致病菌.真菌以白色念珠菌为主,占53%.亚胺培南对鲍曼不动杆菌保持良好抗菌活性,耐药率为12%.结论心脏外科监护病房获得性感染中多重耐药菌株分离率较高,应重视从抗感染药物以外的途径来减少医院感染.

Abstract:Objective To analyze the bacterial pathology and antibiotics resistance in Intensive care unit(ICU). Methods The patients who underwent cardiac operation from September 1999 to June 2004. and infected were involued in this study. The pathogens and the antibiotic sensitivity of them were reviewed retrospectively. Results A total of 607 strains were isolated in which gram-negative bacteria accounted for 49.8%, fungi for 27.8% and gram-positive bacteria for 22.4%. Six main bacterias strains were identified: Acinetobacter baumannii, pseudomose aeruginosa, Staphylococcus aureus, Klebsiella pneumoniae, Enterobacter and Staphylococcus haemolyticus. Candida albicans accounted for 53% in fungi infection. Imipenem was highly active against Acinetobacter baumannii, and its resistance rate was 12% . Conlusions The rate of multi-resistance bacteria isolatied is high in Nosocomial Infection ICU. It is difficult for physicians to choose the appropriate antibiotics. We should focus on other methods to reduce the hospital acquired infection.
