经尿道电汽化术治疗合并尿潴留的前列腺癌

目的 :探讨经尿道电汽化术 (TVP)治疗合并尿潴留的前列腺的疗效。方法 :对 30例合并尿潴留的前列腺癌患者 ,2 0例行TVP治疗 ,10例仅作耻骨上膀胱造瘘术。术后均予以内分泌治疗及 (或 )放射治疗。结果 :术后 1个月 ,TVP组中 10 0 % (2 0 / 2 0 )可自行排尿 ,而膀胱造瘘组中 2 0 % (2 / 10 )可自行排尿。术后 6月时 ,TVP组均可排尿 ,膀胱造瘘组 80 % (8/ 10 )可自行排尿。比较术后 1月和 6月时两组的最大尿流率 (MFR)、国际前列腺症状评分 (IPSS)和前列腺体积均有显著性差异 (P <0 .0 5 )。结论 ;经尿道电汽化术治疗合并尿潴留的前列腺癌 ,可迅速解除膀胱出口梗阻 ,提高患者生存质量。

TRANSURETHRAL VAPORIZATION OF PROSTATE FOR THE TREATMENT OF PROSTATIC CANCER WITH URINE RETENTION

Objective:To evaluate the clinical effect of transurethral vaporization of prostate (TVP) for the treatment of prostatic cancer with urine retention. Methods:Thirty cases of prostatic cancer were devided into two groups: TVP group (20 cases) and suprapubic cystostomy group (10 cases). All patients received endocrinotherapy and (or) radiotherapy after operation. Results:One month after operation, 20 of 20 cases (100%) could void urine spontaneously in TVP group and only 2 of 10 cases (20%) could do it in suprapubic cystostomy group. Six months after peration, all cases could void urine spontaneously in TVP group and 8 of 10 cases (80%) could do it in suprapubic cystostomy group. Compared the TVP group with the suprapubic cystostomy group, the maximum flow rate (MFR), IPSS and the volume of prostate after 1 and 6 months had distinguished differences ( P <0.05). Conclusions:TVP can speedily relief bladder outlet obstruction of prostatic cancer patients, and improve their life qualities.
