基于静息态功能磁共振技术探讨针刺疗法对血管性痴呆患者脑功能的影响

摘    要:目的:基于静息态功能磁共振成像扫描技术,探讨针刺对血管性痴呆患者脑功能的影响。方法:对纳入的15例血管性痴呆患者行针刺治疗与静息态功能磁共振成像扫描,用简易精神状态检查(MMSE)量表评价患者精神功能缺损程度,并用局部一致性方法分析数据,比较患者治疗前后的磁共振扫描结果,分析其与MMSE量表评价结果的相关性。结果:治疗后患者局部一致性(ReHo)增高的脑区有左侧顶下小叶、右侧顶下小叶、右侧顶上小叶、右侧中央后回、右侧中央前回、右侧楔叶/楔前叶、右侧辅助运动区,ReHo值减低的脑区为左侧尾状核/丘脑/苍白球;左侧顶下小叶、右侧顶下小叶、右侧顶上小叶ReHo值与MMSE得分呈正相关。结论:针刺的神经作用靶点主要位于视空间注意网络和感觉运动功能网络、顶叶-前运动区神经环路和基底节-丘脑神经环路,主要功能是提高血管性痴呆患者近距离空间运动功能,改善注意力。

Abstract:Objective:To investigate the effect of acupuncture on brain function in patients with vascular dementia(Va D)using resting functional magnetic resonance imaging(rs-f MRI)scanning.Methods:Fifteen patients with Va D were treated with acupuncture and given rs-f MRI scan,the degree of mental function defect was evaluated by mini mental state examination(MMSE)scale,and the data were analyzed by regional homogeneity(Re Ho).The results of rs-f MRI scan before and after treatment were compared and the correlation with the MMSE scores was analyzed.Results:After the treatment,the patients showed increased Re Ho at the left inferior parietal lobule,right inferior parietal lobule,the right superior lobule,right central posterior gyrus,right anterior central gyrus,right cuneate/anterior cuneate lobe and right auxiliary motor region,and showed decreased Re Ho at the left caudate nucleus/thalamic/palloid,and the Re Ho values of left inferior parietal lobule,right inferior parietal lobule and right superior lobule were positively correlated with the MMSE scores.Conclusion:The neural action target of acupuncture is mainly located in the visual spatial attention network and the sensorimotor function network,the parietal-anterior motor region nerve loop and the basal ganglia-thalamic nerve loop.It’s major function is to improve the spatial motor function and attention of Va D patients.
