再生障碍性贫血患者T细胞CD45RA/RO表达与中医辨证分型的相关性

目的；应用统计学判别分析方法，建立再生障碍性贫血（AA）的中医辨证分型判别方程，探讨T细胞CD45RA／RO与辨证分型的关系。方法：采用流式细胞技术对34例AA患者及34例健康对照者外周血免疫学指标进行检测，同时对AA患者进行辨证分型。结果：筛选出CD45RA^＋绝对值等5个最有意义的免疫指标，组成AA辨证分型判别方程，其判别总正确率为76．5％。与健康对照组比较，CD8^＋ CD^45；RO^＋（％）在AA辨证分型各组中显著上升（P〈0．05或P〈0．01）。结论：AA患者CD8^＋CD^45RO^＋（％）表达显著增高，证实了患者体内T细胞异常活化后转化为记忆T细胞，进而导致骨髓衰竭的免疫学发病机制，说明中医辨证分型确有其客观物质基础。CD45RA^＋绝对值在辨证分型中变化显著，可作为AA中医辨证分型的一个客观指标。

Study on Correlativity of Expression of T-Cell CD45RA/RO with TCM Syndrome Types in the Patient of Aplastic Anemia

Objective:Adopting statistical method to establish differentiating equation for TCM syndrome types of aplastic anemia(AA),and to probe the relationship between T-cell CD_ 45 RA/RO and TCM syndrome types.Methods:Immunological indexes of peripheral blood were determined in 34 cases of AA and 34 healthy persons with flow cytometry,and classification of TCM syndromes was carried out in the patients of AA.Results:The most significant 5 indexes screened such as CD_ 45 RA~+ absolute value,constituted the differentiating equation of classification of AA syndromes,with the total correct differentiating rate of 76.5%. CD_8~+CD_ 45 RO~+ expression(%) in all the AA syndrome types increased significantly as compared with healthy control group(P<0.05 or P<0.01).Conclusion:CD_8~+CD_ 45 RO~+(%) expression in AA patient increases significantly,confirming that after abnormal activation,T-cells in AA patient transform as memory T-cells,and further lead to immunologic pathogenic mechanism of bone marrow exhaustion,indicating that TCM syndrome classification has a definitely objective substance basis. The obvious changes of CD_ 45 RA~+ absolute value in the syndrome classification can be used as an objective index for TCM syndrome type of AA.
