摘    要:目的:研究急性有机磷中毒(AOPP)致循环衰竭患者的血流动力学变化。方法:总结26例AOPP致循环衰竭患者48h内的血流动力学变化,并分析生存者与死亡者的差别。结果:与正常参考均值比较:SVI、LVSWI在48h内显著降低(P%0.01);RVSWI在24h内显著降低(P〈0.01);CI在16h内显著降低(P〈0.01)。与同组2h比较,生存者的CI、SVI、LVswI/RVswI在24h后明显改善(P〈0.05或P〈0.01),血乳酸显著降低(P〈0.05或P〈0.01);死亡者无改善。结论:AOPP致循环衰竭患者的心脏做功能力明显受损,治疗后生存者状况可以改善。

Abstract:Objective:To investigate hemodynamic changes in paitients with circulatory failure induced by acute organophosphorus pesticide poisoning (AOPP). Method: To sum up the hemodynamic changes of 26 patients with circulatory failure induced by AOPP within 48 hours, and to analyze difference between survivors and non-survivors. Result:To compare with normal mean value:Stroke volume index(SVI) and Left ventricular stroke work index(LVSWI) significantly reduced( P〈0.01) ; Right ventrieular stroke work index(RVSWl) significantly reduced within 24 hours(P〈0.01) ;Cardiac index(CI) significantly reduced within 16 hours(P〈0.01 ). To compare with value of same group within 2 hours,CI and SVI and LVSWI/RVSWI of survivors significantly improved after 24 hours(P〈0.05 or P〈0.01),blood lactate significantly reduced(P〈0.05 or P〈0.01);but CI and SVI and LVSWI/RVSWI of non-survivors had not obviously improved. Conclusion: When circulatory failure happened, the working ability of the heart was significantly impaired. Treatment can improve cardiac function of survivors.
