肾透明细胞癌的MRI诊断

目的：探讨肾透明细胞癌的MRI表现，提高对该病的认识。方法：搜集临床资料完整并经手术病理证实的肾脏透明细胞癌25例，均行MRI扫描，其中16例行MRI增强扫描。结果：本组25例患者均为单肾发病，其中左肾11例，右肾14例。肿块直径2．0～16．2cm，其中肿块直径小于4．0cm 8例，直径大于40cm17例。平扫在T1WI呈低信号20例，等信号2例，混杂信号3例，T2M呈高信号22例，低信号1例，混杂信号2例。肿块呈膨胀性生长21例，呈浸润性生长4例。肿块内见不规则坏死、囊变区，MR信号显示不均12例。显示假包膜12例。增强显示16例均以边缘强化为主，全瘤均呈不均匀强化；16例中于皮质期明显强化4例，强化程度高于肾皮质，呈轻中度强化11例，不强化1例；实质期明显强化3例，轻度强化13例；肾盂期16例均呈轻度强化。12例肿块内有不规则坏死、囊变区于增强各期中均未见强化。假包膜呈轻中度持续性强化。结论：MRI能观察肾癌生长方式，了解侵犯周围组织及转移情况，根据信号和增强表现能作出较正确诊断。

MRI diagnosis of clear cell carcinoma of kidney
