扩大翼点入路治疗重度颅脑损伤的临床研究

目的 :探讨扩大翼点入路治疗重度颅脑损伤的临床价值。方法 :52例重度颅脑损伤病人采用扩大翼点入路 ,3 0例采用常规开颅方法。扩大翼点入路即在翼点入路基础上扩大暴露范围 ,包括额底、颞底及额颞顶较大范围。扩大翼点入路结合硬膜扩大缝合 ,有时采用浮动骨瓣等方法。结果 :扩大翼点入路组治愈率高于常规开颅组 (P <0 0 5) ;减少脑组织膨出发生率 (P <0 0 5)、降低再次手术率 (P <0 0 5)等方面也都优于常规开颅组。结论 :扩大翼点入路治疗重度颅脑损伤具有以下优点 :提高治愈率 ,降低脑组织膨出发生率、降低再次手术率

A Clinical Study on Treatment of Severe Craniocerebral Injury by the Amplification Pterion Craniotomy

Objective:To explore the value of treatment of severe craniocerebral injury by the amplification pterion craniotomy Method:52 cases of severe craniocerebral injury were used amplif ication pterion craniotomy,30 cases were used usual craniotomy The amplifica tion pterion craniotomy was included forhead,temple,top Some of those were with dura amplifier issuing,sometimes used floating bone Result:The cure rate of the amplification pterion group was hige r th an that of the usual craniocerebral group,which had lower bone cerebral hernia and lower re-operation rate.Conclusion: The amplification pterion craniotomy treati ng severe craniocerebral injury has advantages:higher cure rate,1ower cer ebral hernia rate,1ower re-operation rate
