川芎嗪注射液对糖尿病肾病患者肾小管功能及肾间质纤维化状态的影响

目的：评价川芎嗪注射液对糖尿病肾病患者肾小管功能及肾间质纤维化状态的影响。方法：将128例糖尿病肾病患者随机分成两组，对照组和观察组各64例，对照组采用基础治疗，观察组在此基础上加载川芎嗪注射液，比较治疗前后两组患者尿N-乙酞-β-D氨基葡萄糖苷酶（NAG）、视黄醇结合蛋白（RBP）、α1微球蛋白（α1-MG）、B2微球蛋白（J32-MG）、IV型胶原（CⅣ）水平和血清透明质酸（HA）、层黏蛋白（LN）、Ⅲ型前胶原（PCⅢ）、Ⅲ型胶原（Col Ⅲ）、尿素氮（BUN）水平，同时测定24h尿蛋白、血浆白蛋白和肾小管对白蛋白的重吸收率。结果：治疗2、4周后观察组尿NAG、RBP、α1-MG、G2-MG、CIV水平和血清HA、LN、PCBI、ColⅢ、BUN水平均显著低于对照组（P均〈0．05）；观察组患者24h尿蛋白低于对照组，血浆白蛋白及肾小管对白蛋白的重吸收率高于对照组，差异均有统计学意义（P〈0．05）。结论：川芎嗪注射液能调节糖尿病肾病患者的尿液及血清相关因子水平，提高肾小管对白蛋白的重吸收率，有效促进肾小管功能及肾间质纤维化状态的改善。

Influence of chuanxiongqin injection on tubular function and renal interstitial fibrosis of patients with Diabetic nephropathy

Objective： To evaluate the influence of chuanxiongqin injection on the tubular function and renal interstitial fibrosis of patients with diabetic nephropathy （DN）. Methods ：128 DN patients were randomly divided into two groups, control group and observation group by half. Patients in control group were treated with basic treatment, patients in observation group with chuanxiongqin injection. Then the NAG, RBP,α1-MG,β2 -MG and CIV levels in urine, HA, LN, PCM, ColⅢ and BUN levels in serum of two groups before and after treatment were compared. During the same time, 24h urine protein, serum albumin and tubular reabsorption rate of albumin were also compared. Results：After 2, 4 weeks of treatment, the urine levels of NAG, RBP, α1-MG,β2 -MG and CIV of observation group were all lower than control group, while the serum levels of HA, LN, PCⅢ, ColⅢ and BUN were also significantly lower than control group （ all P〈0.05）. And 24 h urine protein of observation group was significantly lower than control group, the indexes of serum albumin and tubular reabsorption rate of albumin were all higher than control group （ all P〈0.05）. Conclusion：Chuanxiongqin injection can effectively adjust related factor levels in urine and serum for DN, improve the tubular reabsorption rate of albumin. And it is beneficial to the tubular function and renal interstitial fibrosis.
