缺血预处理对胆管缺血再灌注损伤的保护作用

摘    要:目的观察缺血预处理对胆管缺血再灌注损伤的保护作用。方法24只SD大鼠随机分为假手术(SO)组、缺血再灌注(IR)组、缺血预处理(IP)组,每组各8只。建立大鼠肝外胆道缺血再灌注模型,IP组于缺血再灌注前行一次缺血预处理。各组术毕取下腔静脉血、胆汁及肝外胆管标本。检测血中丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)水平,胆汁中r-谷氨酰转肽酶(r-GT)活性,胆管组织中超氧化物歧化酶(SOD)水平;胆管组织电镜下行病理观察。结果IP和IR组的肝及胆管组织均受损伤,IR组较IP组严重。IR组SOD[(161.71±10.34)U/mgprot]显著低于SO组[(181.19±11.73)U/mgprot]和IP组[(175.23±6.48)U/mgprot](P〈0.05),而ALT[(240.00±102.95)u/L]、AST[(558.31±236.01)U/L]及r-GT[(32.05±14.96)U/L]均显著高于SO组[(97.50±47.02)U/L]、[(239.65±91.72)U/L]、[(11.32±7.25)U/L]和IP组[(145.38±65.32)U/L]、[(372.70±152.59)U/L].[(17.63±7.26)U/L](P〈0.05)。病理观察IP纰亦优于IR组。结论IP对大鼠胆管IR损伤具有保护作用。
