2型糖尿病合并非酒精性脂肪肝舌脉特征研究

摘    要:目的观察2型糖尿病合并非酒精性脂肪肝患者舌脉特征,探讨2型糖尿病合并非酒精性脂肪肝患者的疾病特点。方法通过回顾性统计对照研究的方法,选取2型糖尿病合并非酒精性脂肪肝患者166例作为观察组,选取2型糖尿病不合并非酒精性脂肪肝患者154例作为对照组。观察2组舌质、舌苔、脉象分布情况,并对2组舌质、舌苔、脉象进行比较。结果2组共出现暗红、淡暗、淡红、红、紫暗5种舌质表现。观察组以暗红舌最多,其次为红舌;对照组以暗红舌最多,其次为红舌及淡暗舌。2组共出现白苔、白厚苔、白厚腻苔、白腻苔、薄白苔、薄黄苔、黄苔、黄厚苔、黄腻苔9种苔象表现。观察组以白腻苔最多,其次为薄白苔;对照组以薄白苔最多,其次为薄黄苔。2组共出现沉脉、沉滑脉、沉细脉、沉细滑脉、滑脉、滑数脉、缓脉、细脉、细滑脉、细数脉、弦脉、弦滑脉、弦数脉、弦细脉14种脉象。观察组以弦滑脉最多,其次为弦脉;对照组以细脉最多,其次为沉细脉。红舌观察组丨15例(69.28%),对照组121例(78.57%),观察组较对照组少(P<0_05)。暗舌观察组104例(62.65%),对照组123例(79.87%),观察组较对照组少(P<0.05)。厚腻苔观察组96例(57.83%),对照组54例(35.06%),观察组较对照组多(P<0.05)。滑脉观察组52例(31.33%),对照组32例(20.78%),观察组较对照组多(P<0?05)。细脉观察组73例(43.98%),对照组86例(55.84%),观察组较对照组少<0.05)。结论痰湿可能是2型糖尿病合并非酒精性脂肪肝发生及发展的重要因素,在诊治过程中,应考虑痰湿的影响及注意健脾化湿药物的使用。

The characteristics of tongue and pulse in type 2 diabetes mellitus complicated with nonalcoholic fatty liver

Abstract:Objective To observe the characteristics of tongue diagnosis and pulse in type 2 diabeteswith nonalcoholic fatty liver, and to investigate the characteristics of diseases in patients with type 2diabetes mellitus complicated with nonalcoholic fatty liver disease. Methods By means of retrospectivestatistical control study, 166 patients with type 2 diabetes mellitus complicated with nonalcoholic fattyliver disease were selected as observation group, 154 patients with type 2 diabetes mellitus withoutnonalcoholic fatty liver disease were selected as the control group. The distribution of tongue, tonguecoating and pulse in 2 groups were observed. Results There were 5 kinds of tongue in 2 groups,including the dark red tongue, the light dark tongue, the reddish tongue, the red and the purple dark tongue. The observation group has the largest number of the dark red tongue, followed by the red tongue.The control group had the largest number of the dark red tongues, followed by the red tongues and paletongues. There were 9 kinds of tongue coating in 2 groups, including the white tongue coating, the whiteand thick tongue coating, the white thick and greasy tongue coating, the white and greasy tongue coating,the thin and white tongue coating, the thin and yellow tongue coating, the yellow tongue coating, theyellow and thick tongue coating and yellow and the greasy tongue coating. The observation group had thelargest number of the white and greasy tongue coating, followed by was the thin and white tongue coating;the control group had the largest number of the thin and white tongue coating, followed by was the thinand yellow tongue coating. There were 14 pulse in 2 groups, including the sunken pulse,the sunken andsmooth pulse, the sunken and thready pulse, the sunken thready and smooth pulse, the smooth pulse, thesmooth and rapid pulse,the slow pulse,the thready pulse,the thready and smooth pulse,the thready andrapid pulse, the stringy pulse, the stringy smooth pulse, the stringy and rapid pulse, the stringy and thethready pulse. The observation group had the largest number of the stringy and smooth pulse, followed bystringy pulse. The control group had the largest number of the thready pulse, followed by sunken andthready pulse. The observation group had 115 cases of red tongue (69. 28% ) , while the control grouphad 121 cases (78. 5 1 %) . The observation group was less than the control group ( P < 0. 05 ). Theobservation group was less than the control group (P < 0. 05 ). The observation group had 104 cases ofdark tongue (6 2 .6 5 % ) , while the control group had 123 cases (78. 57% ) ,The observation group wasless than the control group ( P < 0. 05 ). The observation group had 96 cases of thick tongue coating(57. 83% ) , while the control group had 54 cases(57. 83% ) ,The observation group was more than thecontrol group( P < 0. 05 ). The observation group had 52 cases of smooth pulse ( 3 1 . 33% ) , while thecontrol group had 32 cases (20.78%). The observation group was more than the control group ( P <0. 05 ). The observation group had 73 cases of thready pulse (43. 98% ) , while the control group had 86cases ( 55. 84% ) , The observation group was less than the control group ( P < 0. 05 ).Conclusion Phlegm wet may be an important factor in the development and progression of type 2diabetes with nonalcoholic fatty liver disease. In the course of diagnosis and treatment, the effects ofphlegm dampness and the use of spleen and dampness drugs should be considered.
